Here’s an example to help the original poster: At one of the branches of one of the three major oil companies, after the core equipment was restarted following maintenance, it was found that the product quality could not be brought up to standard no matter how much adjustments were made. Two parameters in the product analysis data were above the acceptable levels. After analysis, it was determined that there was a malfunction in the most important distillation column, a double-floating valve column. As for the exact nature of the malfunction, engineers from different departments had varying opinions. One engineer, after careful analysis, concluded that three trays in the column had fallen off. Upon stopping the operation for inspection, it was indeed confirmed that three trays had fallen off, which reduced the separation efficiency. The main reason for this was that the trays were replaced during maintenance, but they were not tightened properly during installation, resulting in them being dislodged once the column started operating again.
