【I】Lockout, Tagout, and Verification Procedures When carrying out the lockout/tagout procedure, it is necessary to follow the steps below strictly. Before starting any maintenance work, the person in charge of the task or their authorized representative must carefully fill out the “Lockout/Tagout Worksheet” in accordance with the requirements. This worksheet is prepared in two copies, to be signed by the personnel responsible for production, electrical isolation, and the control rooms in each workshop. After signing, one copy is given to the person in charge of each workshop, while the other copy is locked up and stored by the department responsible for locking procedures; the locking of electrical equipment is carried out by the electrician on duty. 1. Preparation: Before starting work, the person in charge of the task or their authorized representative identifies the equipment to be locked, prepares the locks, and conducts a locking registration. (Including risk analysis) 2. Notification: Inform the shift supervisors in each workshop as well as those affected. 3. Shutdown: Cutting off power and gas to locked equipment is generally done through the control rooms in each workshop. 4. Isolation: Perform energy isolation on hazardous energy sources, such as turning off circuit breakers or incoming air switches, closing gates, and using potential energy isolation devices. 5. Lockout and tagout: Apply locks to the energy isolation equipment and attach tags. 6. Zero-energy state: Complete release of residual energy. 7. Verification: Ensure that all personnel are in safe positions, then try again to start the locked device. 8. Execute tasks: Perform maintenance and inspections on devices that have been properly locked. 9. Inspection and restoration: Once the work is complete, clean up the site, return any remaining tools, materials, and safety equipment to their proper places, and inform the control rooms in each workshop that the maintenance is finished and the equipment is ready to be started up. 【II】Lock Removal/Tag Removal Procedure: After the locking/tagging process is completed, the locks and tags must be removed. The following steps must be strictly followed when removing the locks/tags: 1. Inspect the area surrounding the equipment to ensure that there are no people or tools in the hazardous zone, and the person in charge of the operation or the area supervisor shall determine the number of affected persons. 2. Check whether all tools have been removed, whether the safety protection devices have been reinstalled, and whether the interlock devices and other components are in a standby state. 3. Inform the relevant technical staff to begin unlocking and retrieving the cards. 4. Verify that all staff members have obtained their safety work cards, and that they fill out the “Lockout/Tagout Worksheet” carefully together with the relevant personnel; this worksheet must be signed by the person in charge, electricians, station workers, and the person in charge of the control room, among others ; This form is made in two copies: one is given to the person in charge of each workshop, and the other is kept on file in the maintenance department. 5. Recheck the environment around the equipment to ensure that there are no people or tools in its hazardous area. 6. Unlock/withdraw the card, restore the energy source, and start the device. 【III】Lockout/Tagout Procedure for Shift Handovers: Shift handovers that involve lockout/tagout must take place on-site (with all staff present). 1. The staff from the previous shift shall inform those affected to remove the locks/tagges. 2. The staff from the two shifts involved in the shift handover and the affected personnel arrived at the scene. 3. The next shift’s staff shall unlock/collect the cards in accordance with the unlocking/card collection procedure, and when filling out the “Locking/Tagging Worksheet,” select “Shift Change.” 4. The successor carries out locking/tagging in accordance with step 4 of the lockout/tagout/verification procedure. These work procedures must be strictly followed! Note: 1. For equipment that requires manual rotation next to the machine, a label must be placed on the “Centralized/Local” control switch; a label must also be placed on the local switch when it is not in use. 2. When shifting shifts but the equipment has not been fully repaired, the locks should be changed simultaneously.
