For a chemical company, it is essential to ensure intrinsical safety; that is, even in the event of operator errors, there should be cascade protection mechanisms to prevent any problems from occurring. 1. Operating equipment and facilities in a faulty condition, as well as disabling the alarm and interlock systems without approval; having alarm systems for flammable and toxic gas leaks in an abnormal state; not activating the emergency shutdown system; inaccurate level gauges; uncalibrated safety valves; inaccurate pressure gauges; and clogged vent valves – all of these issues must be completely resolved, otherwise no matter how good the technical capabilities are, they will be of no use. 2. All personnel must complete training and work with valid certificates; the safety measures specified in the tickets for the eight types of operations must be followed. Giving orders that violate regulations or forcing others to work in dangerous conditions is strictly prohibited. Operations must be carried out in accordance with established procedures. There must be a plan for process handover, inspection after equipment repair, and confirmation before the system is put into use – all of these are aspects of human safety management.
