Everyone who comes to this place can be considered colleagues in this field. In this section, those whose companies have experienced accidents can share those incidents, so as to provide a basis for learning, discussion, and reference; this helps to prevent similar accidents from occurring in other companies, keeping a constant warning sign in mind! A considerable number of accidents are caused by violations of regulations. Although such accidents can be avoided, many people think they are impossible to occur; yet they still happen. By examining specific accidents, people can realize the potential dangers associated with such assumptions, and thus prevent accidents from happening. For example, in the case of heavy oil sampling: although samplers were available, the operator found it troublesome and would instead secretly drain the oil from the vent of the heat exchanger, which was at a temperature of over 300 degrees. He was aware that this was against the rules, but he thought nothing would go wrong; yet during one of these sampling attempts, the oil caught fire on its own, resulting in an accident. Thanks to this accident, there’s no need to emphasize the requirement – from now on everyone will voluntarily use the samplers for sampling, as no one dares to risk their life any longer.
