1. Shandong Province Professional and Technical Title Evaluation Form 2. Assessment forms for each year since assuming the current position 3. Papers and publications that reflect the individual’s academic and professional capabilities since taking on the current position 4. Original certificates of education and degrees 5. Certificates of completion of continuing education courses 6. Current professional and technical title qualification certificate 7. Original appointment letter or official document confirming the appointment 8. One copy of the “Six Publicities” Supervision Card; signatures must not be replaced by stamps. 9. Besides these forms, what else is included in the work summary? Which of these forms need to be filled out by me? I’m a newcomer seeking advice; thank you very much!
