Procedure for reviewing evaluation reports

The municipal work safety supervision bureau organized experts for evaluation, with the work safety supervision bureaus of districts and counties participating. After the remarks by the bureau’s leadership, the matter was handed over to the expert group, with the group leader taking charge of the meeting: first, the company was asked to present its situation; next, the unit responsible for conducting the safety assessment (the specialized report) was asked to explain the contents of the report; finally, the experts discussed the matter and formed their opinions. During this process, the experts asked questions regarding the report, and at the end they signed their names, thus concluding the meeting! ! ! !

Procedures for the filing of safety assessment reports, emergency rescue plans, and information on major hazard sources (I) Procedures for filing safety assessment reports of chemical enterprises 1. The enterprise submits the safety assessment report along with the \"Registration Form for Filing Safety Assessment Reports\" to the local work safety supervision bureau (both the enterprise and the safety assessment agency must affix their official seals)； 2. The county safety supervision bureau assigns relevant personnel to inspect the status of rectification of the potential accident hazards identified in the safety assessment report; once it is deemed satisfactory, the official seal is affixed and the report is submitted to the municipal safety supervision bureau’s office ； 3. The Municipal Work Safety Supervision Bureau’s office shall affix its official seal to effect registration ； 4. The “Safety Assessment Filing Form” is issued in three copies, with one copy kept by the enterprise, one by the district/county safety supervision bureau, and one by the municipal safety supervision bureau’s office. (II) Procedures for filing emergency rescue plans of hazardous chemicals enterprises 1. The enterprise shall submit the emergency rescue plan (one electronic version and one paper copy) along with the \"Registration Form for Filing Emergency Rescue Plans\" (stamped with the enterprise’s official seal) to the district or county work safety supervision bureau ； 2. The district/county work safety supervision bureaus review the emergency rescue plan; once it passes the review, they affix their official seals to it and submit the electronic version of the plan, along with the \"Emergency Rescue Plan Filing Registration Form\", to the municipal work safety supervision bureau ； 3. After approval at the regulatory bureau’s counter, the official seal is affixed for record-keeping ； 4. Three copies of the 《Emergency Rescue Plan Filing Registration Form» are required, with one copy kept by the enterprise, one by the district/county work safety supervision bureau, and one by the municipal work safety supervision bureau’s office. (III) Procedures for the registration of major hazard sources in hazardous chemicals enterprises: 1. The enterprise shall submit to the district/county work safety supervision bureau a \"Application Form for Major Hazard Sources\" (one electronic version and one paper copy), as well as a \"Registration Form for Major Hazard Source Registration\" (stamped with the enterprise’s official seal) ； 2. The district/county work safety supervision bureaus review the \"Application Form for Major Hazard Sources\"; once it passes the review, they affix their official seals to it and submit the electronic version of the application form along with the \"Registration Form for the Filing of Major Hazard Sources\" to the municipal work safety supervision bureau ； 3. After passing the review at the Safety Supervision Bureau’s counter, the official seal will be affixed for record-keeping ； 4. The “Registration Form for the Filing of Major Hazard Sources” is issued in three copies, with one copy kept by the enterprise, one by the district/county work safety supervision agency, and one by the municipal work safety supervision agency’s office.

Generally, after the unit introduction report is presented in the safety assessment (special report), and before the experts hold a discussion to form their opinions, they need to visit the project site as well.

It’s no longer called an evaluation; the State Administration of Work Safety has issued documents prohibiting such evaluations.

First, the evaluating agency provides an explanation of the contents of the report; thereafter, the expert panel reviews the report in terms of both content and format. After that, the experts will go to the site in person to see whether the actual situation matches what is described in the report. Finally, determine whether the report reflects the actual situation on site, formulate expert opinions, and submit them to the Safety Supervision Bureau.

Different evaluations require different procedures: 1. The procedure for safety pre-evaluation is as follows: Depending on the scale of the investment project and the nature of the enterprise, the project owner submits the evaluation report, the report on safety conditions, and the project application to the relevant safety production supervision and management departments (at the national, provincial, and municipal levels). After receiving these documents, the relevant departments organize experts to review the evaluation report, the report on safety conditions, and the project application, and provide their opinions thereon. The evaluation agency then revises the evaluation report, while the project owner revises the report on safety conditions and the project application. Once these revised documents are signed by the head of the expert review team, they are submitted to the relevant safety production supervision and management departments, which issue a document approving the project’s establishment. 2. The procedures for safety acceptance evaluation and status assessment are as follows: Depending on the scale of the investment project and the nature of the enterprise, the project owner submits the evaluation report, application form, and related documents to the relevant safety production supervision and management departments (at the national, provincial, and municipal levels). After receiving these documents, the relevant departments organize experts to review the evaluation report, application form, related materials, as well as the site itself, and provide their opinions on them. Once the project owner has made the necessary corrections, the local safety production supervision department and the safety evaluation agency conduct a follow-up inspection, each issuing a corresponding report. The safety evaluation agency then revises the safety evaluation report. The enterprise submits the relevant documents to the competent safety production supervision and management department, which then issues or reissues the safety production license.
