During the fourth incident, I knew there were gas vents below the area where work was being carried out; despite this, I covered those vents and proceeded with the work, which resulted in a flash explosion and fire. During the fifth instrument maintenance task, which involved heating of the level gauge, the work was not carried out with sufficient care due to the high temperature at the site where welding was performed; as a result, the flange of the level gauge started leaking, and a fire broke out during the welding process. On the sixth occasion, the construction team insisted on carrying out welding work; after putting up shielding, a fire broke out during the welding due to incomplete analysis below and a loose connection on the pressure gauge. On the seventh occasion, the blind plates were not installed properly, the analyzer stopped working, and a fire broke out during welding – at that time, they were on top of the tower. For the eighth time, the area where hot work was being carried out was long, and the valves were leaking fire. Fortunately, I was mostly on site and managed to put an end to it in time
