“There is a story about “the little monk shaving heads” and *inherent violations of rules*. Once upon a time, after becoming a monk, a young monk began to learn how to shave heads. The old monk first had him practice on wax gourds; after each practice session and after shaving, the young monk would stick the razor on the wax gourd. Later, while shaving the old monk’s head, he also stuck the razor on the old monk’s head casually. This story tells us that *bad habits due to inertia are very harmful. In power companies, many accidents are related to bad habits of *inertia; we refer to such behavior at work as “*inertia-related violations.” The main reason for such habitual violations is the lack of a strong safety awareness among those who commit them; they rely on luck and wrongly believe that such habitual violations are not actually violations. What they don’t realize is that these minor violations lay the groundwork for safety accidents and become the cause of disasters. American scholar Hein Xing once analyzed 550,000 various work-related accidents, of which 80% were caused by *involuntary violations of regulations. In electrical safety work, good habits will make our work safer, while bad habits can only harm both others and ourselves. Therefore, each of us must develop good safety habits in working, and must never violate regulations – especially avoiding the habit of routinely breaking rules. Only by starting with ourselves, driving away complacency from our minds and eliminating habitual violations in our work, and by ensuring that the mindset and behavior of strictly adhering to regulations are deeply rooted in us, can we believe that accidents will stay away from us, and that the safety landscape of our company will be clear and secure.
It must be said that* habitual violations pose a great threat; therefore, it is necessary for our safety managers to frequently go to the work sites to conduct inspections, to talk about these issues regularly, and to address them during training sessions. Gradually change this *habit! Of course, economic measures can be used when necessary! ! This leaves a deeper impression!
“To achieve excellence, one must also pay attention to the details. An experienced team leader with over a decade of experience has a thorough understanding of the entire set of equipment and processing techniques; he is adept at dealing with abnormal situations, demonstrating great experience and high-level skills. During a pump switch, I saw how quick and decisive he was – the valve was secured in no time. But once the pump started, it failed to build pressure. So the pump was stopped and the valve was checked again; it turned out that the inlet valve of that pump, which should have remained almost open, had actually been closed. Another example: an expert DCS operator who handles DCS systems with ease, and of course is also well-versed in the pipeline networks, valves, and instrument tags at the plant site. One day, during a night shift, the team leader asked her, who was just at the scene, to turn on all the lighting fixtures in the area; this posed a challenge for her as she couldn’t find the switches. It was only with the help of a patrol officer that she was able to locate them. Although these two examples involve minor issues and did not cause any disruptions in production or accidents under those circumstances, if viewed from a different perspective, the outcomes would be quite different. If the senior supervisor starts the pump and then receives an order to deal with some abnormal situation immediately, he may not pay attention to the pressure of the pump, which can result in the pump being damaged and the system experiencing abnormalities ; If there was an urgent need for electrical power for lighting in order to address a major leak in a device, and the DCS expert could not find the switch, it would delay the repair of the leak, which could lead to an accident. The above two things are clear examples of excellence in one’s work rather than being concerned with trivial details. The reasons for this can be summed up as follows: those who are highly skilled consider themselves to be experienced after having been through many challenges; they believe they can handle major issues without any trouble, so minor matters are not worth worrying about ; Those who are expert in their field believe that it is sufficient to excel in their own role; there is no need to understand things that are not closely related to their job. Indeed, expertise is very important in petrochemical production, as it helps to turn major accidents into minor ones, and minor issues into nothing at all ; Because \"vigor\" can also nip impending accidents in the bud. But the ‘minor details’ in chemical production are equally important as well. Some small actions may seem trivial on the surface, and might even be overlooked inadvertently, but they can hide great risks; once the conditions are right, they can lead to serious consequences. As the old saying goes, “Problems arise from what is overlooked, and disasters start with small things.”” ; “A dike a thousand miles long can collapse because of an ant hole. Safe production is a task that requires strictness, meticulousness, and practicality; it allows no room for negligence or carelessness. Every stage and every step in the production process must be handled with care and caution. Having only expertise without paying attention to details will lead to futile efforts; therefore, to achieve true expertise, one must also pay attention to details.
Support! Experience is a double-edged sword; operational experience is important, but falling into empiricism while operating can also be terrifying! And empiricism is only committed by operators with rich experience! As everyone just said, safe production requires constant attention – there must be no moment of relaxation! I hope that all chemical industry professionals make use of their experience at work, avoid relying on empiricism, and let their extensive experience assist them in carrying out production and design tasks!
