First, check the design, then procurement, and then manufacturing. If there are problems in any of those stages, address them accordingly
It’s hard to say; the equipment exploded and nothing was left. All that remains is the design.
It would have been possible to check the procurement details, but in today’s society, even the lists of materials purchased and the certificates of material quality are fake.

When an accident occurs in a container, it is necessary to investigate the causes from various perspectives. However, it is often difficult to identify causes related to processes or operations; for example, if an operator’s mistake leads to overpressure in the equipment and there are no other records besides those from the pressure gauge (which is common in factories), and the operator denies responsibility, then it becomes hard to determine the cause. At this point, even if there are minor defects in design or manufacturing, the responsibility cannot be shirked. Therefore, those of us involved in the design and manufacturing of containers must be extremely careful and not make any mistakes on the drawings, not even the slightest one.
