The organization has a range of preventive measures, but they are not always implemented properly; if technical controls can be put in place, accidents can be avoided. For example, since we work with oil products, we require drivers to park the tank truck in the designated location, then remove the car keys and hang them in a specified place (to prevent the driver from turning on any non-explosion-proof equipment inside the vehicle during loading and unloading operations, or from starting the vehicle without permission while personnel are still working inside it). There’s another issue: before starting the oil dispensing process, the operator failed to insert the hose properly (it is required to be no more than 20 centimeters from the bottom), or didn’t insert the hose at all, while someone else had already started the dispensing procedure. What good measures can be taken in regular operations to avoid such violations?

The two people upstairs are right; it is undeniable that the operator’s sense of responsibility comes first. But the current situation is whether it is possible to use technical means for control in order to achieve essential safety and reliability. Because a person's condition is influenced by many uncertain factors, such as family, physical health, and treatment.
You and one of my client owners are in the same situation: both of you want to address the sense of responsibility related to safe operations through technical means. But it’s also necessary to take into account that self-control isn’t a panacea, and even if such technical solutions do exist, is it economically feasible? Sometimes, when automation is too high, the sense of responsibility among operators decreases significantly!
Accidents occur as a result of unsafe human behavior, unsafe conditions of objects, or management deficiencies. However, a person’s performance is influenced by many uncertain factors, such as family situation, health status, weather conditions, and treatment received. If the operator is not in good health and is taking certain medications, the probability of him making mistakes increases significantly (mistakes of that minor nature would not occur under normal circumstances). If we can exert control through the process, then even if someone engages in unsafe behavior, the process will not allow it – oil will not be supplied or the vehicle will not start – thereby preventing accidents from occurring in the first place. Wouldn’t that be better? Additionally, there are factors such as people’s sense of responsibility, safety awareness, and safety training, as well as certain institutional constraints – it’s not possible to dismiss or reassign them. It’s quite difficult to avoid problems from a human perspective; therefore, it’s better to implement controls through equipment and processes. The above are my personal opinions; I welcome everyone to discuss them together.
