Sharing of accident handling experience: I believe many people working in fertilizer plants encounter various accidents or incidents. When dealing with such accidents or incidents, there are many lessons to be learned. Compared to knowledge from books, the experience gained from others' handling is more practical and useful. Perhaps it can bring some inspiration or useful insights to oneself. After all, not everyone has many opportunities to handle accidents, and this valuable experience in dealing with them proves to be extremely useful when we encounter accidents ourselves. It is recommended to combine the two patches, and everyone is encouraged to participate in the updates to gradually improve them.
Process accident: Explosion of the pressure gauge on stage 6 of the compressor. Incident details: During the reverse operation of the compressor, while air was being supplied to stage 6, one of the six outlet valves was closed, while the other valve was shut off due to an operator’s mistake – the vent valve was closed without the supply valve being opened, resulting in immediate overpressure on stage 6. This led to the explosion of the pressure gauge on that stage and subsequently a fire. Incident handling: Emergency shutdown. Incident analysis: A serious accident caused by operational error. Precautions: When the compressor is operated in reverse, the on-duty operators must have clear roles assigned and work under unified command.
