How could such a tank be crushed by pecking?

During the production process, accidents can occur in which the tanks get flattened, and this mostly happens with tanks under negative pressure. However, accidents where a tank at normal pressure and equipped with a respirator gets crushed by pecking are quite rare, right? I wonder how such a tank can be crushed by pecking?
If the respirator is large enough yet gets flattened by pecking, how should this be analyzed?
The breather valve was installed incorrectly: lol The intake valve was used in place of the exhaust valve, or a double-acting valve was used instead of a single-acting one; as a result, only exhaust occurs and no intake takes place: lol
If there was no problem originally, I have encountered this situation where the breathing valve gets clogged. We also experienced the situation where the tank collapsed during discharge, and it was only later that we discovered the problem of a clogged vent valve.
1. Check whether it is equipped with a single-port valve or a double-port valve. 2. Check whether the breathing valve is blocked. 3. Whether the breather valve is suitable for the actual operating conditions, or whether it is not completely blocked but its flow rate is already very low. 4. Even under normal pressure conditions, nitrogen supplementation is required to ensure stability.
