What remedial actions are required if the openings are made incorrectly in a pressure vessel?
Solution: 1. Redo it; 2. Seal off by welding a blind plate ; 3. Just leave it as is – keep it as a spare hole; who knows it might come in handy someday, hehe.
1. If the location of the incorrect hole does not coincide with the new correct hole location, and there is sufficient distance between the two holes to meet the standard requirements for spacing between adjacent welds: Consult the process engineer to ensure that no fluid stagnation areas will arise that could affect product quality, and also consult the PDS model engineer to confirm that there will be no interference with other systems such as pipelines, instruments, or insulation; in such cases, this area can be designed as a connection port equipped with a blind flange; 2. If any of the above conditions cannot be met, then the following factors shall be considered: a. Whether the owner will accept re-welding the groove at that location using the originally cut shell plates, carrying out full penetration welding, grinding the weld surface to be level with the shell surface, and performing 100% RT inspection; the acceptance standard shall be in line with the highest requirements for Class A and B welds on the shell ; At the same time, 100% MT/PT inspection must be carried out on the weld surface, with grade I as the acceptance standard (MT should be used whenever possible for carbon steel equipment) ; b. If the owner, the pressure vessel inspection agency, or the third-party inspection party do not accept the above solution a, then it is necessary to cut off a section of the cylinder that includes this defective hole; the length of this cut-off section must be at least 300 mm, and the distances from both ends of this cut-off section to the adjacent uncut ring seams must also be at least 300 mm. This section is then replaced with a newly manufactured cylinder, after which a new hole is drilled ; The flaw detection requirements for all welds (including those added) shall comply with the requirements of the original drawings
If possible, discuss with the customer the option of creating a backup port
