Looking for accident cases related to improper operation of hand valves?

There are manual valves in front of and behind the safety valve on the flare system; they are usually kept open (unlocked). Have there been any incidents where such valves were accidentally closed? Or what about accident cases resulting from misoperation involving valves that are not used frequently?
In design, consideration must generally be given to the system’s safe pressure relief in cases of blockages upstream and downstream (mainly due to valves closing accidentally).
