About reforming heat exchangers

In our unit, the elbow of the heat exchanger for the reforming and pre-hydrogenation inlet/outlet material exploded twice in two years. What was the cause? Are there any good preventive measures?
Heat exchanger elbow explosion? Is the pressure drop in the heat exchanger too large to hold back the pressure?
Is it the same place where the explosion occurred, whether it was at the feeding point or the discharging point?
Severe corrosion leading to leakage and explosion
Reply 1# sdmt1984 Is there a dechlorination reactor for pre-hydrogenation? When we did not have a dechlorination reactor, pipeline elbow rupture in low-temperature areas and corrosion and leakage of air coolers occurred.
The temperature is high and the gasket seal is not tight, which may be a reason.
