I. Submission of relevant documents: 1. Application form for change of registration； 2. Practicing Certificate for Registered Safety Engineers of the People’s Republic of China (original certificate) ； 3. Copy of the labor contract signed with the original employer, or proof of termination of employment ； 4. Labor contract or employment document (copy) signed with the new employer, as well as employment certificate ； 5. Documentation issued by the former employer regarding the applicant’s performance during the period of employment ； 6. 1 recent 2-inch color photo of the applicant without a hat, taken face-on (to be attached to the application form) ； 7. “Summary Table of Applicants for Changing Registration of Registered Safety Engineers” (electronic document). II. Requirements for submitting materials: (1) Fill out the “Application Form for Change of Registration of Registered Safety Engineers” as required (see attachment). Except for the signature, which should be written in a pen or signature pen, all other information filled in by the applicant should be printed on A4 paper.
