On June 4, 1987, Ying, a maintenance worker in the finishing workshop of a vinylon factory belonging to a petrochemical company in Shanghai, inhaled formaldehyde while repairing valves. Feeling unwell a day later, he went to the hospital where he was diagnosed with formaldehyde poisoning. Cause of the accident: Violation of safety operating procedures, lack of awareness and training regarding safety and hygiene, and failure to use personal protective equipment
