Analysis of the reasons for quadlog ESD interlock shutdown

The ESD used in the boiler system of our plant is the QuadLog from MOORE Company. A shutdown incident occurred recently, and upon checking the system’s SOE records, it was found that the first interlock signal recorded was a DO signal rather than a DI signal. Have any of you encountered similar situations? What could be the possible reasons?
It is possible to have a situation where, after comparing two analog values, a DO signal is generated; a stoppage can be caused by analog signals, and not all stoppages necessarily result from DI signals – it depends on how interlocks are implemented in your program.
