China Petroleum & Chemical Corporation Occupational Safety and Health Management System (Part) (Sinopec [1995] An Zi No. 237, May 24, 1995) (1) Accident Management System Chapter 1 General Provisions Article 1 In order to promptly report, count, investigate and handle accidents, and actively take preventive measures to prevent accidents, this system is specially formulated. Article 2 This system is based on * * * The "Regulations on Reporting and Handling of Enterprise Employee Casualty Accidents" are formulated and apply to all enterprises within the Petrochemical Corporation system. Article 3 The reporting, statistics, investigation and handling of accidents must adhere to the principles of seeking truth from facts and respecting science. Chapter 2 Classification and Nature of Accidents Article 4 Classification of Accidents 1. Fire Accidents: During the production process, fires occur due to various reasons, resulting in casualties or material and property losses. 2. Explosion accident: During the production process, explosions occur due to various reasons, causing casualties or property damage. 3. Equipment accident: Accidents causing damage to machinery, power, telecommunications, instruments (meters), containers, transportation equipment, pipelines and buildings (structures) due to design, manufacturing, construction, use, maintenance, management, etc. 4. Production accidents: Accidents such as production stoppage (reduction), material leakage, and material misalignment were caused due to violations of operating rules and regulations, command errors, and power outages, water outages, steam (gas), and air outages. 5.Traffic accident: Accidents that cause casualties, damage to vehicles and ships, and loss of materials and property due to violations of traffic and shipping rules and mechanical equipment failures while vehicles and ships are traveling. 6. Personal accident: During the working process at the production post, in addition to the above five types of accidents, there are production-related casualties or acute poisoning accidents that occur to the registered employees of the enterprise. Article 5 The nature of the accident shall be classified according to the severity and nature of the accident into: 1. If an enterprise-level accident meets one of the following conditions, it is an enterprise-level accident.: (1) The accident resulted in minor injuries, no serious injuries, and no deaths. (2) The direct economic loss caused by an accident is less than 10,000 yuan (excluding 10,000 yuan). (3) An accident causes the loss of oil (material) to be less than 10 tons (excluding 10 tons). No matter how much is recovered, the loss amount shall prevail (the same below). (4) An accident caused the shutdown of 1 to 2 production units, affecting daily output by 50% or more. 2. Report a general accident to the head office. Any general accident that meets one of the following conditions is a general accident to be reported to the head office.: (1) An accident caused serious injuries to 1 to 2 people, but no deaths. (2) An accident causes a direct economic loss of 10,000 yuan or more and less than 150,000 yuan (excluding 150,000 yuan). (3) An accident causes a loss of oil (material) of more than 10 tons and less than 50 tons (excluding 50 tons). (4) An accident involving one person causes the shutdown of three or more production units, affecting 50% or more of daily output. 3. Report a major accident to the head office. Any major accident that meets one of the following conditions is a major accident to be reported to the head office.: (1) An accident seriously injured 3 to 9 people. (2) An accident kills 1 to 2 people. (3) An accident causes direct economic losses of 150,000 yuan and more but less than 6 million yuan (excluding 1 million yuan). (4) An accident caused the loss of more than 50 tons of oil (material). 4. If a serious accident is reported to the head office and meets one of the following conditions, it is a serious accident reported to the head office.: (1) An accident causes serious injuries to 10 or more people. (2) An accident killed 3 to 9 people. (3) An accident causes direct economic losses of 1 million yuan or more and less than 5 million yuan (excluding 5 million yuan). 5. Report a serious accident to the head office. Any serious accident that meets one of the following conditions is a serious accident to be reported to the head office.: (1) An accident causes the death of 10 or more people. (2) An accident causes direct economic losses of 5 million yuan or more. Chapter 3 Accident Reporting Article 6 After an accident occurs, the parties involved or the person who discovered the accident should immediately report it directly or level by level to the person in charge of the enterprise. Fire accidents should be reported to the fire alarm first. Article 7 After receiving the accident report, the person in charge of the enterprise shall, according to the nature of the accident, report the time, place, circumstances, consequences, preliminary analysis of the causes, measures taken, etc. to the competent department of the head office by telephone, telegram or fax (using "Accident Express") for all accidents reported to the head office within 24 hours after the accident. Major accidents involving personal casualties should be immediately reported to the labor department, public security department, people's procuratorate, and labor union where the enterprise is located. Article 8 In the event of a casualty or poisoning accident, the accident scene should be protected and necessary measures should be taken quickly to rescue the injured and prevent the accident from expanding. When a major fire and explosion accident occurs, an on-site command center should be formed with the leaders of the enterprise to provide unified command and handling to prevent the accident from spreading. Article 9 If an accident occurs and is reported to the head office, a formal accident report must be written and reported to the head office department within 30 days after the accident occurs. When reporting the accident report, the enterprise shall submit the "Three Don't Let Me Go" registration form for accident handling together with the report. Article 10 The enterprise safety supervision and management department shall report the accident summary of the unit last month to the competent department of the head office by telegram or fax on the 1st of every month. Submit the monthly accident report (the monthly report includes the "Enterprise Accident Overview Summary Table" and the "Enterprise Accident Comprehensive Statistical Table") to the head office department before the 6th of each month. Chapter 4 Accident Investigation Article 11 If an enterprise-level accident occurs, a general accident is reported to the head office, or a major accident is reported to the head office, the enterprise shall organize an investigation ; A serious accident occurred and was reported to the head office, and the head office department participated in the investigation ; If a serious accident occurs and is reported to the head office, the head office will investigate together with relevant local departments. Article 12 The safety technical supervision and management department of an enterprise is the centralized management unit for various accidents and is responsible for the summary, statistics, analysis and reporting of various accidents. The investigation departments responsible for various types of accidents are divided as follows:: 1. The safety and technical supervision and management department is responsible for the investigation of personal accidents, fire accidents and explosion accidents. 2. The investigation of equipment accidents shall be the responsibility of the equipment competent department. When equipment damage leads to production shutdown accidents, the equipment management department should be the main investigation, and other departments should cooperate. When an equipment accident involving personal injury or death occurs, the safety technical supervision and management department should be the main investigation, and the equipment management department should cooperate. 3. The investigation of production accidents is the responsibility of the production department. 4. The investigation of traffic accidents is the responsibility of the traffic authorities. Article 13 If an accident occurs to Party B of an outsourced project, Party B shall be responsible for organizing the investigation and handling. Accidents involving one or more deaths, three or more serious injuries, and direct economic losses of RMB 150,000 or more should be reported to the head office for filing. 14th Accidents that occur in collective enterprises affiliated to various units shall be investigated and handled by the collective enterprises and their competent departments. ; Any injury or death accident that occurs to employees of a collective enterprise participating in the labor of a national enterprise shall be investigated and handled by the national enterprise organization. ; Casualty accidents that occur to temporary workers and migrant workers shall be investigated and handled by the enterprise where the accident occurred. The above-mentioned accidents shall be reported to the safety and technical supervision and management department, and separate statistics shall be reported. Article 15 Traffic accidents that occur when employees use the company's transportation vehicles to perform company tasks outside the company or take the company's commuter cars or boats on their way to and from get off work will be investigated by the transportation department and statistics will be based on the company's employee casualty accident reports. Article 16 Members of the accident investigation team shall meet the following conditions: 1. Have expertise in a certain aspect required for accident investigation. 2. Has no direct interest in the accident that occurred. 3. Have the character of being serious, responsible, and seeking truth from facts. Article 17 Responsibilities of the accident investigation team 1. Find out the cause, process, casualties and economic losses of the accident. 2. Determine the person responsible for the accident. 3. Provide accident handling opinions, preventive measures and suggestions. 4. Write an accident investigation report. The content of the report should be clear, accurate and concise. Article 18 Accident economic losses include direct economic losses and indirect economic losses. 1. Direct economic losses: Expenses incurred after personal injury or death, aftermath costs and value of property damage. The losses incurred by employees due to work-related injuries shall be calculated according to the statistical methods prescribed by the local labor department. ; Fire losses are calculated according to the fire loss calculation methods of the Ministry of Public Security. ; Vehicle and ship losses are calculated according to the local insurance company’s loss compensation. If the equipment is damaged and cannot be repaired or used, it shall be calculated based on the net book value of the fixed assets. ; If the equipment can be repaired and used, the repair cost shall be calculated based on the actual damage. Material loss should be calculated based on the company's material plan price. Loss of products (semi-finished products) resulting in scrap should be calculated according to the company's planned cost ; Rework should be calculated based on the actual loss after rework and the loss of materials after processing (including water, electricity, steam) ; If the standard is lowered and shipped from the factory, the actual difference shall be calculated. 2. Indirect economic losses: Suspension of production and loss of production value. The production suspension period is calculated from the occurrence of the accident to the full resumption of formal production. Losses from multi-system production shutdowns should be calculated based on the planned costs of each enterprise. Chapter 5 Accident Handling Article 19 Accident handling must adhere to the principle of "three no-misses", that is, if the cause of the accident is not clearly analyzed, no one will be let go. ; Those responsible for the accident and the masses were not educated and were not spared. ; No precautions are taken. Article 20 If an accident occurs due to neglect of production safety, illegal command, illegal operation, violation of labor discipline, dereliction of duty, or failure to take effective measures when discovering potential accident hazards or dangerous situations, or failure to actively deal with them, the competent department of the enterprise or the enterprise shall be ordered to * * Relevant regulations stipulate that administrative sanctions will be imposed on the person in charge of the enterprise and those responsible for the accident. ; If a crime is constituted, criminal liability shall be investigated by judicial organs in accordance with the law. Article 21 After an accident occurs, anyone who conceals or fails to report, makes a false report, deliberately delays in reporting, deliberately destroys the scene of the accident, or refuses to accept investigation or provide relevant information and information without justifiable reasons shall be punished by the relevant departments in accordance with the regulations. * * Relevant regulations stipulate that administrative sanctions shall be imposed on the person in charge of the relevant unit and the directly responsible personnel. ; If a crime is constituted, criminal liability shall be investigated by judicial organs in accordance with the law. Article 22 The following situations must be dealt with seriously: 1. Those who are irresponsible at work, violate labor disciplines, fail to strictly implement various rules and regulations, and are the main responsible persons for causing accidents. 2. Those who are mainly responsible for accidents caused by failure to implement safety technical measures as scheduled and failure to take emergency measures. 3. Give instructions that violate regulations, force risky operations, and dissuade those who are mainly responsible for the accident. 4. Those who are mainly responsible for accidents caused by neglecting working conditions and reducing safety protection measures. 5. The person who is primarily responsible for accidents caused by long-term disrepair of equipment, malfunctioning operation, and failure to take emergency measures. 6. After an accident occurs, the person who fails to handle the situation according to the "three no-passes" principle, fails to learn lessons seriously, and fails to take corrective measures is the main person responsible for causing the accident to reoccur. Punishment shall be carried out in accordance with the "Regulations on Reward and Punishment of Enterprise Employees". Article 23: Punishment Approval Authority 1. Enterprise-level accidents and general accidents reported to the head office shall be handled by the enterprise. 2. If a major accident occurs and is reported to the head office, it will be handled by the enterprise and reported to the head office. 3. If a malignant or serious accident occurs and is reported to the head office, the enterprise shall put forward handling opinions and report it to the head office for handling. 4. If the accident is serious, the superior department has the right to handle it directly. Chapter Six: Relevant Provisions on Accident Reporting Article 24: Any major, malignant or extremely serious accident that is reported to the head office shall be reported to the head office in accordance with the regulations. Article 25 Reporting personnel should report major, malignant or extremely serious accidents to the head office: The person in charge of the company directly under the company, the safety director and the main person in charge of the unit where the accident occurred. A serious accident occurs: The manager (factory director) of the directly affiliated enterprise, the safety director and the main person in charge of the unit where the accident occurred. Article 26 The time requirement is to report to the head office as soon as possible based on a basic investigation of the main reasons for the accident, but the time shall not exceed 15 days. Article 27 Reporting Materials Prepare an accident report (including the accident process, accident causes, liability analysis, preventive measures, accident handling opinions and relevant investigation materials, etc.), accident scene photos and accident scene videos. Article 28 Units and Personnel to Listen to Reports 1. If an accident involving production or equipment is involved, the production department is requested to send personnel to attend. If an accident involving a construction company occurs, please send personnel from the engineering department to attend. If there is an accident involving the sales system, please send personnel from the sales company to attend. 2. Vicious accidents and major accidents: Report to the leaders of the head office. In addition to the above-mentioned relevant personnel, please send personnel from the Human Resources Department and the Bureau of Discipline Inspection and Abuse to participate. Article 29 Reporting Procedures 1. The enterprise where the accident occurred reports the accident situation. 2. Participants jointly analyze the causes and lessons learned from the accident. 3. Leaders put forward opinions and requirements. Article 30 The head office shall establish reporting records of major, malignant and extremely serious accidents and archive them for future reference. Chapter 7 Supplementary Provisions Article 31 Each enterprise shall formulate its own implementation details in accordance with the requirements of this system. Attachment (omitted) (2) Safety Inspection System Article 1 Safety inspection is one of the important means to establish a good safety environment and production order and do a good job in safety work. We must adhere to the principles of combining leadership with the masses, combining general inspection with professional inspection, and combining inspection with rectification, and ensure that it is institutionalized and regular. Article 2 To carry out safety inspections, there must be clear purposes, requirements and specific plans, and a safety inspection organization that is responsible for leaders at all levels and participated by relevant personnel must be established. Article 3 The contents of safety inspections include inspection of leadership, inspection of ideas, inspection of disciplines (including labor discipline, process discipline, work discipline, and construction discipline), inspection of systems, inspection of violations, and inspection of hidden dangers of accidents. Article 4 The head office shall organize annual inspections or spot checks on the safety work of directly affiliated enterprises. A major job responsibility system inspection with safety inspection as an important content is organized by the company (headquarters) every six months ; The factory organizes once a quarter ; Workshops are organized monthly. Article 5 Safety inspections adopt four inspection methods: daily, regular, professional and irregular. During professional inspections, the competent departments will take the lead and relevant units will be organized to participate. Article 6 Daily safety inspections shall be carried out as follows:: 1. Team leaders and workers in production positions should strictly perform shift handover inspections and mid-shift inspections: 2. Team leaders and workers in non-production positions should conduct inspections before and during work based on the characteristics of their positions. ; 3. Leaders at all levels and safety management personnel at all levels should, within their respective business scopes, frequently go to the shipyard to conduct safety inspections, discover unsafe problems, and urge relevant departments to solve them in a timely manner. Article 7 Regular safety inspections include seasonal inspections and pre-holiday inspections. 1. Seasonal inspections and spring safety inspections focus on lightning protection, anti-static, anti-thaw leakage, and building collapse prevention. ; Summer safety inspection focuses on heatstroke prevention, typhoon prevention, and flood prevention ; Autumn safety inspection focuses on fire prevention, antifreeze and heat preservation ; The winter safety inspection focuses on preventing fire, explosion, gas poisoning, freezing, condensation and slipping. 2. Inspection before the holiday: Check safety, security, fire protection, production preparation, backup equipment, etc. before the holiday. Article 8 Enterprises should conduct professional inspections of boilers, pressure vessels, electrical equipment, mechanical equipment, safety equipment, monitoring instruments, dangerous goods, protective equipment, fire-fighting facilities, transportation vehicles, dust and virus prevention, liquefied gas systems, etc. every year. Article 9 Enterprises should conduct unscheduled safety inspections before the equipment is started and shut down, during repairs, when new equipment is completed and during trial operation. Article 11 Inspection organizations at all levels must study and prepare rectification plans for the identified hidden dangers one by one, and achieve the "three determinations" and "three disobediences" (three determinations): Determine measures, appoint responsible persons, and set deadlines for completion. “Three no friendships”: If the team can rectify it, it will not be handed over to the workshop; if the workshop can rectify it, it will not be handed over to the factory; if the factory can rectify it, it will not be handed over to superiors). Article 12 For items that cannot be rectified temporarily, in addition to taking effective preventive measures, they should be included in technical measures, safety measures or maintenance plans, and rectified within a time limit. Article 13: For major safety hazards discovered during inspections at all levels, each unit must implement the "Hidden Hazard Rectification Notice" for management. The content of the "Hidden Hazard Rectification Notice" includes hidden hazard items, rectification opinions and resolution deadlines. The notification form is handled by the safety technical supervision department and is issued after being signed by the factory director in charge of safety when necessary. The person in charge of the unit and department where the hidden danger is located signs for it and is responsible for handling it. Otherwise, if an accident occurs, you will be held accountable according to the circumstances. The notice must be archived and kept for future reference. (3) Safety Production Guarantee Fund Management Measures Chapter 1 General Provisions Article 1 The petrochemical industry is * * One of the important pillar industries, doing a good job in safe production is related to protection * * property, protecting the lives and safety of employees and increasing economic benefits. In order to strengthen disaster prevention and loss prevention work to reduce and avoid disaster accidents, and to enable enterprises to receive timely economic compensation and quickly resume production when they suffer property losses caused by natural disasters or accidents, the economy * * * Approved to establish a production safety guarantee fund (hereinafter referred to as the fund) within the China Petroleum and Chemical Industry Corporation (hereinafter referred to as the company). Article 2 The management and use of the head office’s security funds shall be the responsibility of the head office’s safety supervision office (hereinafter referred to as the head office’s safety supervision office). Anhui Industrial Company, under the leadership of the Safety Supervision Office of the head office, is responsible for the collection, preservation and appreciation of security funds, and improves the safety technology and safety equipment level of the head office system through the development of the security technology industry. Article 3 The establishment of a security fund within the head office is an economic * * * Specially approved form of intra-industry insurance: All enterprises and institutions affiliated to the head office shall pay security funds to Anhui Industrial Co., Ltd. managed by the safety supervision office of the head office in accordance with the provisions of these management measures, and shall not insure from other insurance companies. Article 4 The head office entrusts Sinopec Huaxia Audit Co., Ltd. to audit and supervise the surrender of security funds by affiliated enterprises and institutions, the return of security funds, the return of security funds, and the use of various subsidy funds. Each enterprise must pay in full, earmark the funds for special use, and accept audits. Chapter 2 Payment of Security Fund Article 5 The following properties of the enterprises and institutions affiliated to the head office are within the scope of payment of security fund. 1. Fixed assets and inventories. 2. Complete sets of equipment introduced in capital construction projects. 3. The new device has been completed and put into trial operation, but the fixed assets have not yet been reported for production. 4. Other insurable property. Article 6 The following properties are not within the scope of payment of the security fund: 1. Fixed assets include land, roads (excluding factory roads), transportation vehicles (excluding construction machinery and tools), and ships (excluding barges). 2. Materials and entrusted processing materials during transportation. 3. Temporary temporary installation works. Article 7 Joint-stock enterprises or Sino-foreign joint venture projects in which enterprises and institutions directly under the head office hold shares (or investment amount) exceed 50% or control (including 50%) shall be fully insured with Anhui Industrial Company. If the above proportion is less than 50% or does not hold a controlling stake, in addition to ensuring that the security funds are paid in proportion to their own shareholding proportions or investment amounts, the enterprises and institutions directly affiliated to the head office should also guide joint-stock enterprises or joint venture projects to fully insure with Anhui Industrial Company from the perspective of conducive to safety management and safeguarding corporate interests. Article 8 Collective enterprises formed by enterprise units affiliated to the head office shall, in principle, obtain insurance from the head office. Article 9 The withdrawal standards and insurance period of the security fund. 1. Fixed assets are withdrawn based on the original value of fixed assets (deducting the original value of fixed assets specified in Paragraph 1 of Article 6), and inventory is withdrawn based on 3‰ of the average book balance in the last 12 months (deducting materials in transit and entrusted processing materials specified in Paragraph 2 of Article 6). The average inventory balance is: The payment basis for the previous year was based on the average of the monthly closing balance from January to December of the previous year minus the deductions. The next year will be based on the average of the outstanding balance minus deductions for each period from July of the previous year to June of this year. Pay in two installments each year: For the first time, 50% of the withdrawal standard is paid before March 1 of this year. The insurance period starts from 0:00 on March 1 to 24:00 on August 31. ; For the second time, 50% of the withdrawal standard shall be paid before August 15 of this year. The insurance period starts from 0:00 on September 1 to 24:00 on February 28 of the next year. 2. When a new device is completed and put into commissioning, before it is reported for production (that is, the fixed assets have not been transferred), 85% of the adjusted investment budget after deducting items outside the insurance scope and items to be transferred out and amortized shall be used as the base. According to 3‰ of the base, the payment shall be made in two installments each year. The payment time is the same as above. The insurance period starts from the date of commissioning and ends with the date of transfer. After the insurance expires, it will be calculated based on the actual number of insurance days, and any excess will be refunded or less will be compensated. * * After acceptance, the provisions of paragraph 1 of this article shall be followed. 3. Complete sets of equipment introduced in capital construction projects should be insured based on the CIF price of the equipment at an annual rate of 1.5‰. Payment should be made in two installments each year, and the payment time is the same as above. The insurance period can be from the date of purchase or from the time of delivery to the construction site until the new device is completed and put into trial operation. After the insurance expires, it will be calculated based on the actual number of insurance days, and any excess will be refunded or less will be compensated. After the new device is completed and put into trial operation, the provisions of paragraph 2 of this article shall apply. 4. Insurance measures for other properties not covered by the first three paragraphs of this article may be formulated separately by referring to the first three paragraphs and negotiated between the enterprise and the head office’s safety supervision office. Article 10 When the security fund enters the cost in one go, if it causes cost fluctuations, it can be amortized in installments. Article 11 In order to understand the details of the insured property and facilitate rate determination and claim settlement, each insured enterprise shall fill in a security fund account every year based on the original value of assets and the average balance of inventory at the end of December of the previous year, and submit it to the Safety Supervision Office of the head office before March 1 of each year, with a copy to Anhui Industrial Company. Article 12 Every time the enterprise pays the security fund, it must fill out a security fund payment form and send it to Anhui Industrial Company with the payment. Anhui Industrial Company will review and confirm based on the remittance voucher, payment form and ledger, and return the security fund receipt as the official certificate for the company to participate in internal insurance. Chapter 3 Use of Security Fund Article 13 When an enterprise suffers from natural disasters and accidents within the scope of insurance liability and causes loss of insured property, Anhui Industrial Company shall be responsible for claims settlement in accordance with the relevant regulations of the head office. If the amount of compensation is more than 1 million yuan, Anhui Industrial Company will investigate and verify it, put forward a compensation opinion, report it to the head office's safety supervision office for review, and then submit it to the head office's safety supervision committee for approval. Article 14 In order to maintain and increase the value of the security fund, for directly-affiliated enterprises that pay the security fund in full and on time throughout the year, that is, for all directly-affiliated enterprises that pay the security fund in full before March 1 and August 15 every year, the head office will return 17% of the security fund paid to the enterprise. For enterprises that fail to pay the security fund in full on time, the head office will return 15% of the security fund paid. The returned funds are managed by the safety department of the directly affiliated enterprise and used for various expenses related to safety production. Article 15 The use of security funds must ensure funding for major scientific research projects carried out for production safety. Article 16 The head office's reward fees for advanced units in production safety can be spent from the security fund in accordance with the relevant reward regulations of the head office's safety supervision committee. Article 17 The major accident hazard rectification projects declared by the enterprise shall be reviewed and approved by the Safety Supervision Committee of the parent company after being formally evaluated by experts organized by the Safety Supervision Office of the parent company or a designated consulting company. The required head office subsidy funds shall be withdrawn from the security fund within the approved amount. Chapter 4 Supplementary Provisions Article 18 "China Petrochemical Corporation's Internal Insurance Clauses for Corporate Property" and "China Petroleum & Chemical Corporation's Internal Insurance Claims Rules for Corporate Property" are supplements to these Measures. Article 19 The right to interpret these regulations and the above terms and conditions belongs to the Safety Supervision Office of the head office. If there are any matters not covered, the right to interpret these regulations and the above terms and conditions shall not be * * In addition to handling the spirit of relevant laws, decrees and relevant documents of the parent company, the Safety Supervision Office of the parent company is responsible for revising and supplementing them, and reports them to the Safety Supervision Committee of the parent company for approval. Article 20 These Measures shall come into effect on June 1, 1994. The old method will be abolished with immediate effect. If the contents of the security fund in documents issued by the head office in the past conflict with the provisions of these Measures, the provisions of these Measures shall prevail. Article 2 These regulations apply to the new construction, expansion, reconstruction and major technological transformation projects of petrochemical enterprises within the scope of Sinopec Corporation, as well as industrial test equipment for scientific research and development. Article 3 Construction projects must comply with * * Relevant regulations and standards on occupational safety and health, as well as occupational safety and health technical measures and facilities in construction projects, should be designed, constructed and put into operation at the same time as the main project (hereinafter referred to as the "three simultaneities"). When introducing devices and equipment from abroad, supporting occupational safety and health facilities should be introduced at the same time or supplemented domestically. Article 4 The occupational safety and health supervision of construction projects in these management regulations refers to the safety and health supervision department of the construction unit based on * * Laws, regulations, standards, and occupational safety and health supervision throughout the entire construction project process. Article 5 The whole process of occupational safety and health supervision of construction projects is divided into five stages, namely: Project feasibility study report, preliminary design, review of overall construction plan, confirmation of safety conditions before construction, and completion acceptance. Chapter 2 Organization and Management Article 6 The whole process of occupational safety and health supervision of the construction project shall be organized or authorized by the head office and organized or participated by the safety, health, fire protection and other departments of the construction unit, and shall be carried out according to the * * Relevant regulations for invitation places * * Relevant departments participated and worked with local * * Relevant departments shall do a good job in cooperation and coordination. Article 7 Safety and health supervision of construction projects shall be subject to hierarchical management. 1. The investment amount is more than 200 million yuan, * * For approved key construction projects, the safety supervision office of the head office shall participate in the whole-process supervision (according to the specific circumstances of the construction project, the safety supervision office may also entrust relevant departments to participate in specific supervision work). Representatives of the safety supervision office shall sign the review opinions of the competent department and the acceptance opinions of the superior department of the construction unit on the preliminary design review form and acceptance review form. 2. For construction projects with an investment amount of more than 50 million yuan and less than 200 million yuan, the safety supervision office of the head office shall entrust the occupational safety and health department of the directly affiliated enterprises to participate in the whole process supervision. The directly affiliated enterprises shall report to the safety supervision office of the head office in a timely manner in stages. ; The representative of the occupational safety and health department of the directly affiliated enterprise signs the review opinions and acceptance opinions, and reports them to the safety supervision office of the head office for record. 3. Construction projects with an investment amount of less than 50 million yuan and approved by directly affiliated enterprises shall be organized and reviewed by the construction unit itself, and the relevant departments of the construction unit shall participate in the supervision of the whole process. The representative of the occupational safety and health department signs the review opinions and acceptance opinions. 4. For scientific research and development of industrial test equipment, projects with an investment of more than 30 million yuan and fire hazard Class A equipment, the safety supervision office of the head office shall participate in the whole process supervision. The representative of the Safety Supervision Office signs the review opinions and the acceptance opinions of the superior competent department of the construction unit. Industrial test devices for scientific research and development outside these conditions shall be supervised, reviewed and signed by the directly affiliated enterprises themselves. Chapter 3 Supervision Procedures Article 8 When preparing a feasibility study report, the construction unit shall simultaneously demonstrate and evaluate the occupational safety and health content of the proposed construction project. In accordance with the requirements of the hierarchical management provisions of these regulations, it shall entrust a qualified consulting unit to provide evaluation opinions, which shall be submitted after review and approval by the occupational safety and health supervision department. Article 9 The design unit shall prepare a "Special Chapter on Occupational Safety and Health" (including fire protection and disaster prevention content) during the preliminary design stage. During the preliminary design review, the review department shall conduct a review together with the occupational safety and health supervision department at the same level. The construction unit shall fill in the "Construction Project Occupational Safety and Health Preliminary Design Approval Form", and after review by the occupational safety and health supervision department at the next higher level, submit it to the local labor department for approval. Article 10 The construction unit shall, in accordance with the "Petrochemical Construction Project Production Preparation and Commissioning Work System" of the parent company, prepare an "Overall Construction Startup Plan" that includes occupational safety and health content, and submit the occupational safety and health part of the "Overall Construction Startup Plan" to the superior occupational safety and health supervision department for review. Article 11 Before the commissioning of materials for a construction project, under the unified organization or authorization of the head office, the construction unit shall conduct the "Confirmation of Safety Conditions before the Start of Construction" to check whether the safety and health facilities and measures in the "Occupational Safety and Health" have been completed, and whether the conditions for safe commissioning of materials are met. Only after approval by the superior occupational safety and health supervision department can the project be commissioned for commissioning. Article 12 During the completion acceptance stage, the construction unit shall submit the "Completion Acceptance Report" to the occupational safety and health supervision department. After review, it shall be submitted to the local labor department for approval. The report must include "Occupational Safety and Health", "Occupational Safety and Health Acceptance Review Form" and calibration records during trial production of safety, health and fire protection facilities. Those who do not meet the requirements will not be accepted. Chapter 4 Working Methods Article 13 When supervising the occupational safety and health process, supervisory departments at all levels shall use the "Three Simultaneities" checklist to implement supervision in accordance with the provisions of hierarchical management. Article 14 Sinopec Corp.’s “Three Simultaneities” Checklist for Occupational Safety and Health is based on * * Technical documents of regulatory nature prepared with the parent company’s norms, standards, systems, regulations, etc. regarding occupational safety and health. Article 15 At each stage of the whole-process supervision of a construction project, the department responsible for the "three simultaneous" supervision of occupational safety and health shall inspect item by item according to the "three simultaneous" inspection sheets at each stage, and carefully fill in the "three simultaneous" inspection notices at each stage and send them to the construction unit, design unit and local government. * * Labor Department. Article 16 The construction unit and the design unit shall make rectifications according to the requirements of the "Three Simultaneities" inspection notice, and report the rectification information to the "Three Simultaneities" supervision department, and inspect them together in the next stage of the "Three Simultaneities" supervision and inspection. Chapter 5 Veto Article 17 The safety and health supervision department has veto power in the whole-process occupational safety and health supervision of construction projects. Article 18 Construction projects that fail to conduct safety and health supervision and review at each stage in accordance with the supervision procedures of these Measures or that fail to pass the review shall not be reported for project establishment, start of construction, trial production and acceptance. Article 19 If an accident or serious occupational hazard occurs in a construction project that is forcibly constructed and put into operation without review by the occupational safety and health supervision department, the leadership who approved the construction and put into operation shall be held accountable. (5) Regulations on Accident Hazard Management Work Chapter 1 General Provisions Article 1 In order to implement Sinopec Corporation’s safety production policy of “safety first, prevention first, all-hands-on-deck, comprehensive management”, strengthen the management of accident hazard management work, improve the safety level of the enterprise, and enhance the ability to withstand major malignant accidents and natural disasters, these regulations are specially formulated. Article 2 These regulations apply to the accident hazard management projects of petrochemical enterprises within the scope of Sinopec Corporation. Chapter 2: Scope of Accident Hazards Article 3: Unsafe Factors Endangering Production Safety. Article 4 Hidden dangers in production facilities and safety facilities that lead to the occurrence or expansion of accidents. Article 5: Labor environment that may cause occupational diseases or occupational poisoning. Chapter 3 Assessment Methods of Accident Hazards Article 6 LEC Assessment Method: The LEC method is currently used to assess hidden dangers in petrochemical systems. This assessment method comprehensively considers three factors: the possibility of accidents in each link of the production system (L), the frequency of personnel exposure to these environments (E), and the severity of the cows produced in the event of an accident (C). Each factor is assigned a different score (see Appendix 1), which is scored according to the actual situation. The potential hazard of the working environment is evaluated by multiplying the LEC value. The greater the LEC value, the greater the degree of hazard of the hidden hazard. Article 7 Hidden Hazard Assessment Methods: 1. Enterprises should first conduct self-evaluation. The evaluation team is composed of engineering technicians and veteran workers who are led by the company's supervisor and have practical work experience. They conduct in-depth on-site inspections and carry out scoring and rating based on the LEC evaluation method in a realistic and realistic manner. The evaluated projects should establish complete and complete archives, including evaluation reports, review opinions, technical conclusions made by the evaluation team, hidden danger management plans and budget estimates, etc. 2. Based on the self-assessment results, the enterprise's safety department shall prepare an accident hazard management plan for the enterprise for the next year after soliciting opinions from the planning, finance, production, and mobility departments. It shall be approved by the enterprise's manager (factory director) or chief engineer and submitted to the safety supervision office of the head office. 3. The Safety Supervision Office of the head office will conduct a preliminary review of the accident hazard management plans reported by the enterprise, and classify the hidden hazard management projects that comply with these regulations. The Safety Supervision Office will * * Relevant experts or qualified security consulting companies shall be organized to carry out re-examinations on hidden dangers at the level and the head office level, and submit re-examination assessment reports. 4. Based on the review and evaluation report of the accident hazard management project, make an annual plan for the accident hazard management project of the head office and submit it to the safety supervision committee of the head office for approval. Chapter 4 Procedures for Preparing Plans for Accident Hazard Control Projects Article 8 For any hidden hazard control projects that form fixed assets, the technical transformation department of the enterprise shall incorporate safety technical measures into the unified management of investment scale in accordance with the "Interim Management Measures for Technical Transformation of Sinopec Corporation". Projects above the quota shall be approved by the Planning Department of the Corporation. If subsidies from the security fund of the Corporation are required * * Level and head office level hidden danger projects must be reported to the head office safety supervision office for approval at the same time. ; Enterprise-level accident hazard management projects worth more than 1 million yuan must be reported to the safety supervision office of the head office for filing. Article 9 For any hidden danger control projects that fall within the scope of approval by the superior competent department or apply for subsidies, the enterprise should first go through the approval procedures and then include them in the plan. The same procedures should be followed to cancel or adjust the control projects or plans that have been submitted for approval. Article 10 Before the end of July every year, the enterprise's safety department shall negotiate with the technical department to include the hidden danger management projects for the next year in the enterprise's technical measures proposal plan and participate in the pre-arrangement of the technical transformation plan of the planning department of the head office. Article 11 Enterprises shall submit for approval the hidden danger management plan for the next year before the end of October each year. Article 12 New unplanned hidden danger management projects shall be separately declared in accordance with these regulations. Chapter 5: Classification of Accident Hazard Control Projects Article 13: Based on the characteristics of the petrochemical industry and the experience in managing accident hazards, hidden hazards are divided into five levels according to the degree of danger. Accident hazard management projects shall be managed by the enterprise safety department, and management files such as hazard assessment, governance completion status, and effect assessment and acceptance shall be established. 2. In the rectification of various accident hazards, the "four determinations" (determination of rectification plans, funding sources, project leaders, and rectification deadlines) and "three no-push" (what can be solved by the team and work section should not be pushed to the workshop, what can be solved by the workshop should not be pushed to the factory, and what can be solved by the factory should not be pushed to the company or the main factory). 3. For accident hazards that cannot be rectified temporarily, enterprises must take reliable safety measures and strengthen supervision. 4. The main leaders of the enterprise must bear primary responsibility for the rectification of hidden accident hazards in the unit, and the technical person in charge of the enterprise shall be responsible for the feasibility and rationality of the rectification plan for hidden accident hazards. Article 15 Implementation of hidden accident hazard management projects: 1. The enterprise shall comprehensively organize and implement the unit's accident hazard management project and complete the annual plan according to schedule. 2. The head office’s safety supervision office is responsible for * * Supervise, inspect and coordinate the implementation of accident hazard management projects at the corporate level and the head office level. Article 16 Acceptance of accident hazard items 1. * * The safety supervision office of the head office or the relevant entrusted unit shall organize the acceptance of the hidden danger management projects at the level and the head office level. 2. The company (main plant, factory) will organize the completion acceptance of accident hazard projects at or below the company (main plant, factory) level. 3. Within two months after the trial operation of the completed hidden danger control project is basically normal, the engineering department or unit shall report it to the enterprise's safety technology department, organize an assessment and acceptance according to the accident hazard management authority, submit a completion acceptance report, fill in the completion acceptance form, and submit it to the safety supervision office of the head office together with the financial final accounts of the subsidy project. 4. After the project is accepted, the workshop (department) should formulate corresponding rules and regulations and organize operators to learn * , transfer to normal maintenance management. 5. The completion status of enterprise accident hazard management projects must be reported to the safety supervision office of the head office on a quarterly and annual basis. The quarterly report is due before the 5th of the first month of the next quarter, and the annual report is before the end of January of the following year. Chapter 7 Subsidy for Accident Hazard Control Project Funds Article 17 Scope of Subsidy: Reviewed by relevant experts from the Safety Supervision Office of the Head Office or a qualified safety consulting firm, and approved by the Safety Supervision Committee of the Head Office * * Enterprises can submit subsidy applications for level- and company-level hidden danger management projects and submit them to the Safety Supervision Office for review and balance, and subsidies will be provided in accordance with regulations. Article 18 Subsidy standards: 1. * * For projects with level 1 hidden dangers, a subsidy of 30% of the total investment in the project budget will be provided ; 2. For hidden danger projects at the head office level, a subsidy of 20% of the total investment in the project budget will be provided. Article 19: Hidden hazard control projects using subsidy funds shall first use self-raised funds. The head office subsidy funds shall be allocated in installments according to the actual progress of the project, and subsidy funds for multi-year projects shall be arranged annually. Article 20 does not fall within the scope of subsidies: 1. Hidden dangers in facilities (equipment) that are not insured by Anhui Industrial Company ; 2. Hidden danger items of units that fail to pay the security fund in the prescribed amount ; 3. Office facilities, living welfare facilities, and facilities not directly related to production ; 4. Hidden dangers discovered within 3 years of the official commencement of new construction, expansion, and reconstruction projects ; 5. It is an overhaul, renewal, or expansion project (whether it is a complete set of equipment, a single piece of equipment, or a building). Chapter 8 Supplementary Provisions Article 21 The Safety Supervision Office of China Petrochemical Corporation is responsible for the interpretation of these regulations.
