Basic approach to troubleshooting in chemical plants

I would like to ask the more experienced process engineers: if there is a process issue that needs to be resolved in your factory, from which aspects do you usually start to consider things in order to find the root cause or a solution? I just heard that there are considerations related to people, machines, and the environment; it seems to be the SIX SIGMA method! I wonder what everyone’s approach to solving this problem is? Before solving a problem, one must first find out the cause of it. That is, problem (incident) analysis. Analysis can be conducted from five aspects: people, machinery, materials, methods, and the environment. Person: There’s no need to explain this, right? Machine: Equipment. Material: Goods or supplies. Method: Approach, procedure. Ring: Environment.
