Cause analysis: 1. The staff member in question lacked a sense of responsibility, failed to provide proper supervision, did not fulfill their duties properly, and was unaware of the status of the maintenance work. 2. The work tickets prepared by the electricians are not standardized, and safety measures are inadequate. 3. The shift leader fails to conduct thorough inspections and reviews, being careless and negligent.
