Dear friends: Those who have prepared risk assessment reports for category III pressure vessels, could you provide one in a similar format? Thank you.
The regulatory guidelines only specify three types of situations that require a safety risk assessment; the method for conducting such assessments should vary from one organization to another, as the guidelines don’t provide any details on this matter. I don’t want the ASME standards to be written in such detail. In fact, there are many such ambiguous expressions in GB specifications and standards.
Appendix 2 Basic Requirements for Risk Assessment Reports 1 General Provisions 1.1 This section specifies the basic requirements for risk assessment reports. 1.2 The container designer shall prepare a risk assessment report for the anticipated conditions of use of the container in accordance with relevant regulations or the requirements of the design client. 1.3 Designers should fully consider the failure modes that may occur in containers under various operating conditions, and establish safety measures in aspects such as material selection, structural design, and manufacturing inspection requirements to prevent potential failures. 1.4 The designer shall provide the container users with the information necessary to develop emergency response plans for container accidents. 2 Formulation of principles and procedures 2.1 Risk assessment during the design phase focuses primarily on hazard identification and risk control. 2.2 Risk assessment during the design phase is carried out in accordance with the following procedure: a) Determine various operating conditions of the container based on the user’s design requirements and other design input information ; b) Identify hazards based on the medium, operating conditions, and environmental factors in various usage scenarios, to determine the possible hazards and their consequences ; c) For all hazards and corresponding failure modes, specify the safety protection measures to be taken and the basis therefor ; d) Provide the information necessary to develop emergency response plans for potential failure modes ; e) Prepare a comprehensive risk assessment report. 3 Contents of the risk assessment report The risk assessment report shall include at least: a) The basic design parameters of the pressure vessel: pressure, temperature, material, properties of the medium, and external loads, etc ; b) Description of operating conditions ; c) Hazards that may occur under all operating and design conditions, such as explosions, leaks, damage, deformation, etc ; d) For failure modes that are already specified in the standard, indicate the relevant clauses of the standard ; e) For failure modes not specified in the standards, explain the basis for selecting the loads, safety factors, and corresponding calculation methods in the design ; f) Measures for dealing with minor leaks, large-scale outflows, and explosions of the medium ; g) Specify appropriate personal protective equipment and measures based on the potential risk of injury to those around ; h) The risk assessment report shall bear signatures consistent with those on the design drawings.
