Tube blockage ； 2. Blocked central tube ； 3. Lower elbow plug ； 4. Clogged exhaust pipe ； 5. Breakage of the explosion-proof panels on the shell side and in the explosion-proof area ； 6. Tube leakage ； 7. Head leakage ； 8. Leakage in vapor-phase and high-pressure water safety valves ；
Leakage at the upper and lower end caps and the intermediate connection flange.
Cracks in expansion joints, cracks in the welds between heat exchange tubes and tube sheets.

(1) Tube blockage； (2) Clogged central tube ； (3) 3203-C bottom elbow plug ； (4) Clogged exhaust pipe ； (5) Shell-side explosion-proof plate rupture ； (6) Explosion of partition in explosion-proof space and rupture due to tube blockage and pressure ； (7) Remove the HV3202 sleeve in the explosion-proof area that is damaged ； (8) Tube leakage ； (9) Leaking safety valve of temperature control water ； (10) Leakage in head 3203-C.
