Failure to follow scientific methods can lead to accidents. Managers cannot be on site 24/7, but when an accident occurs, it are the workers who suffer first, while the managers might still be sleeping at home. If an accident occurs and someone dies, they are the murderers. The explosion in the urea synthesis tower at the Hebei Qianan Fertilizer Factory resulted in 10 deaths on the spot. I was puzzled – why wasn’t any lesson learned from that?
