Automatic increase in wastewater COD

In our plant, the COD level after the aerobic tank is around 150, but sometimes it rises to around 180 after treatment. Since the water passes through a secondary sedimentation tank, a mixed reaction tank, and a coagulation sedimentation tank, the COD level should decrease over time – so why does it sometimes increase instead?
Could it be that too much chemical was added during the coagulation and sedimentation process, contributing to the COD level? That’s just my humble opinion
