Submission of spare parts requests

Does anyone have a form for requesting spare parts for workshops? Could you send it to me? I’m new to this field and don’t know much about equipment – pumps, heat exchangers, etc. Anything related to valve seals would also be helpful. Thank you!
We all have a material declaration form; one fills in the models of the parts and components, as well as the relevant parameters required. There are also relevant standards, but no specific forms or anything of the kind. To know what needs to be declared, it’s necessary to be familiar with those relevant standards or to look them up
Everything needs to be verified by checking relevant information; it is recommended to purchase from the original manufacturer from the start, so that the manufacturer can provide a list of spare parts along with the technical specifications
