The Measures for the Declaration of Occupational Hazards in the Workplace have been in effect since November 1, 2009. Article 2 Production and business entities (excluding coal mining enterprises) that have or generate occupational hazards within the territory of the People’s Republic of China shall, in accordance with **relevant laws, administrative regulations, and the provisions of these measures, promptly and truthfully report such occupational hazards, and submit themselves to the supervision and management by the departments responsible for work safety oversight. Article 4: The declaration of occupational hazards is subject to hierarchical management based on the location. Production and business operations entities shall, in accordance with relevant regulations, conduct tests and evaluations on the occupational hazard factors in their workplaces, and submit reports to the work safety supervision and management department at or above the county level where they are located, in line with their respective responsibilities. Article 5: When applying to identify occupational hazards, production and business units shall submit the \"Application Form for Occupational Hazards in the Workplace\" along with the following relevant documents: (1) Basic information on the production and business unit ; (II) Information on the production technologies, processes, and materials that generate occupational hazard factors ; (III) Types, concentrations, and levels of occupational hazard factors in the workplace ; (IV) Number of persons exposed to occupational hazards in the workplace and their distribution ; (5) Provision of occupational hazard protection facilities and personal protective equipment ; (VI) Management of employees exposed to occupational hazard factors ; (7) Other materials specified by laws, regulations, and rules. Article 6: The declaration of occupational hazards in the workplace can be submitted in both electronic and paper formats. Production and operation units shall submit electronic data through the \"Workplace Occupational Hazards Declaration and Filing Management System\". Meanwhile, after stamping the \"Workplace Occupational Hazards Declaration Form\" with the official seal and having it signed by the principal person in charge of the production and operation unit, it shall be submitted, together with relevant documents, to the local work safety supervision and management department in accordance with Articles 4 and 5 of these regulations. Article 8: Occupational hazards in the workplace shall be reported annually. Where there are significant changes in the following aspects of a production and operation entity, it shall file a change application with the original registration authority in accordance with the provisions of these rules: (1) In the case of new construction, renovation, expansion, technical transformation, or introduction of technology, an application shall be filed within 30 days as of the date of completion acceptance of the construction project ; (II) If significant changes occur in the occupational hazard factors originally declared and related details as a result of changes in technology, processes, or materials, a declaration must be submitted within 15 days from the date of such changes ; (III) If there is a change in the name of the production and operation entity, or in its legal representative or principal responsible person, a declaration must be submitted within 15 days from the date of the change.
