Manhole on the container

I think everyone knows that the manhole on the equipment is a place for people to enter. What I want to ask is, was this manhole designed with the equipment in mind, or was it created with the need for people to enter it into consideration? Are there some devices that don’t require a person to be inside them during use, but have access holes designed for ease of welding during manufacturing?
Manholes are provided for the assembly, maintenance, inspection, and cleaning of equipment. I don’t think manholes are created merely for the convenience of welding during manufacturing; after all, discontinuities in the cylinder body can lead to stress concentration, so it’s not advisable to add manholes just for ease of welding...
