There is a severe lack of supervision in process management; the team’s technicians, safety officers, supervisors, and the client’s project oversight personnel have all failed in their duties! In the case of such issues with formwork support systems, it is necessary to check: (1) whether the subcontracting arrangements for the project are compliant, whether there is any illegal subcontracting, and whether the construction unit or the labor service unit (specialized subcontractor) meets the required qualification standards ； (2) Whether the staffing levels of the general contractor, supervisor, and subcontractors meet the requirements, whether the personnel are on duty, and whether those in these positions hold the necessary qualification certificates ； (3) Is there a specific construction plan, and has it been approved (is the signature genuine)? Concrete formwork projects over 5 meters in length are considered high-risk projects – was expert evaluation conducted? The special construction plan for high-risk projects must be reviewed and signed by the technical supervisor of the construction unit, and stamped with the unit’s official seal; it must also be reviewed and signed by the chief supervision engineer, who shall stamp it with their professional seal before it can be put into implementation. It is necessary to verify whether the content of the plan is correct and reasonable ； (4) Has the plan been explained? ； (5) Was construction carried out at the site in accordance with the plan? ； (6) Whether inspection was carried out after construction, whether the inspection was successful, and whether any required corrections were made if so ； (7) Whether the construction workers have received safety and technical instructions, and whether supervision is provided during the construction process ； (8) Whether personnel have received safety training, whether those engaged in special operations hold the required certificates, and whether safety rules and regulations are in place and properly implemented ； (9) Check whether the above measures have effectively prevented such issues, whether reports were made, and whether there were any cases of illegal instructions given ； (10) Has the construction unit posted the name of the high-risk project, the construction schedule, and the specific persons in charge in a prominent location at the construction site, and installed safety warning signs in the hazardous areas? Once you check, there will definitely be a bunch of problems
