Summary of Work for 2017: In 2017, all members of the catalysis workshop, with the care and support of company leaders at all levels, adhered closely to the safety production policy of \"safety first, prevention foremost, and comprehensive management.\" They focused on improving internal management and strengthening basic operational procedures, which enabled smooth progress in various aspects related to safety, environmental protection, processes, and equipment within the workshop. Certain achievements were made as a result. The summary of work for 2017 is as follows: I. Work Accomplishments (1) Training and Education Management 1. A yearly safety training and education plan, along with safety activity plans for each team, was developed. In accordance with company requirements, regular safety training sessions were held at least once a month. Training quizzes were prepared, and tests were conducted for all employees, with the results recorded in their individual training records. It also organizes various teams to carry out safety activities on a regular basis, evaluates the effectiveness of each activity, and gradually improves the impact of these safety activities. To date, the workshop has organized fourteen workshop-level safety training sessions, covering topics such as standards for the discharge of wastewater and exhaust gases and emergency response procedures, training on explosions in catalytic chimneys, identification and analysis of hazards associated with catalytic processes and measures for protection against them, training on the newly issued management systems for the catalytic workshop, specialized training on supervision tasks in the catalytic workshop for 2017, training on the JHA and SCL analysis methods, the responsibilities related to safe production, videos on safety accidents, the company’s emergency response plans, the current regulations of the group and the company, as well as the standardized operating procedures for the workshop. Through training, employees’ safety awareness has been greatly improved; no work-related accidents or safety violations occurred in the workshop throughout the year, which has significantly ensured the safe operation of the equipment. 2. To ensure the safe conduct of inspection and maintenance work on the plant, the workshop gathered relevant information and developed training materials on plant startup, shutdown, and inspection/maintenance. Employees in the workshop were given specialized training, covering various safety precautions during shutdown operations, special requirements for work during maintenance, and the verification of safety conditions prior to startup. Training was conducted step by step, in line with the overall workflow, to enhance employees’ awareness of safe working practices, thereby ensuring smooth plant shutdown, safe maintenance, and successful restart – without any incidents involving risky operations. 3. From equipment inspection and maintenance to routine upkeep, as well as the application of fireproof coatings in the second half of the year, there is a constant flow of external workers in the workshop throughout the year. To enhance safety education for these workers, they are informed about the rules upon entering the site, and before each type of special operation, they receive detailed instructions on the safety procedures and potential risks, along with emergency measures. Supervisors are present throughout the operations to ensure safety and thus prevent any accidents related to such work. (II) Task and Document Management 1. Throughout 2017, the catalysis workshop strictly adhered to all the company’s operational regulations, implementing various safety measures on site. The relevant permits issued were as follows: more than 316 permits for work at heights, more than 197 permits for work in confined spaces, more than 2 permits for work involving disconnection of electrical circuits, and more than 359 permits for hot work. By adhering to strict requirements and diligently implementing the company’s various management regulations, not a single accident occurred during special operations, thus contributing to the company’s safe production. 2. On-site process adjustments and other related tasks form the basis for the proper operation of the equipment. To ensure the safety of employees while carrying out these tasks, the workshop implements a system of providing safety instructions for non-routine operations. In addition to routine tasks such as sampling and inspections, safety information is provided by team leaders and workshop staff during shift changes. For any other tasks that involve higher levels of risk, workshop managers or team leaders must organize team members to conduct risk analyses, identifying potential risks and hazards throughout the operation process. Based on these analyses, preventive measures are devised and recorded in task briefing sheets, which are then reviewed and signed off by the workers before the tasks can be carried out safely. The implementation of this measure effectively ensures the thorough execution of the company’s “Five-Step Safety Approach”. By implementing this system, it is possible to ensure employees’ safety during work to the greatest extent possible and raise their awareness of work safety, with excellent results. 3. Through the safety observation and communication activities carried out by the company, it was found that employees did not have a thorough understanding of the work procedures, and there were potential hazards in the work process. By combining these findings with the standard operating procedures for on-site work in the workshops, employees were organized to re-examine these standardized procedures, receive training, and have the content and requirements of those procedures as well as safety precautions improved. This approach helps to avoid various hazards that may arise during daily work. So far, the first batch of procedures has been reviewed, and training and assessments have been conducted. 4. Establish a management ledger for the HSE equipment and facilities related to emergency shutdown and gas handling systems, and maintain and update it regularly. Records of flange tightening and static electricity bonding inspections have been added; these facilities are inspected and maintained at least once a month to ensure they are in good condition and ready for use in emergencies. 5. The monthly training records for the workshop are organized and archived in accordance with the company’s requirements; video materials are added, and they are bound into volumes based on classification. This creates a complete set of records that includes the training content, sign-in sheets, grade sheets, exam papers, and video footage from the training process, thus meeting the company’s standards better. 6. Review accident cases related to catalytic units, such as the \"8.17\" fire accident at Dalian Petrochemical and the \"8.10\" accident at Zhongjie Petrochemical. Based on the causes of these accidents and the actual conditions of the workshops, formulate preventive measures. These measures should be distributed to all employees so that they can learn from the accidents and understand the protective measures in place, thereby preventing similar incidents and ensuring the safe operation of the units. (III) On-site management: 1. Odor control has become a top priority in daily management. Since entering the workshop, efforts have been made to identify and address the sources of odors on site. First, the openings of the hidden drains connected to the surface water were sealed off and marked, in order to prevent the volatilization of oils and gases from those drains. Secondly, new requirements have been set for the sampling process: first, the sampling buckets on site must contain oil, and this oil must be poured out after sampling is completed ; Secondly, the dumping openings of underground oil tanks must be covered to prevent the emission of unpleasant odors ; Third, use leak detection equipment to conduct thorough inspections of areas with unusual odors, identify the source of the leakage, establish corrective measures, and carry out follow-up checks. Through these measures, the sources of unpleasant odors at the site have been brought under control, laying the foundation for the environmentally friendly operation of the equipment. 2. To better track the progress of various maintenance tasks and to arrange personnel and resources more effectively, the workshop has established specific requirements: internal maintenance meetings are held during lunch breaks, during which each responsible person reports on the progress achieved in the morning. Any issues or matters that require decision-making by the workshop are discussed and resolved immediately. At 5:30 p.m., a meeting was held between the workshop and the construction team to discuss the work completed that day, the plans for the next day’s tasks, as well as the preparation and coordination of materials that had not yet arrived. Through meetings held twice a day, communication between the workshop and the contractors, as well as between the workshop and its teams, has become increasingly smooth, allowing work to proceed in an orderly manner and **improving work efficiency. 3. The workshop management staff implement a self-inspection system during their shifts; each worker on duty must conduct a self-inspection of the workshop area each time they are on shift, and the results of these inspections are shared within the workshop. At the end of the month, an HSE improvement meeting is held to summarize and compare the performance of various teams in terms of site management. By bringing to light any issues identified during inspections, this encourages competition among teams, thereby improving site management. To date, the workshops have identified over 3,000 such issues, all of which have been effectively addressed to ensure safety at the plant sites. 4. As required by the company, I reviewed the drawings and design specifications to establish technical requirements that meet ** standards. I coordinated the renovation of the explosion-proof doors in the joint control room, worked together with the Materials Procurement Department on the bidding process, and coordinated with the suppliers to ensure prompt production; the items have now arrived and are ready for installation. 5. The units at the North Plant have been taken out of use; the workshop has filled them with nitrogen for protection, and blind flanges have been installed at the boundaries of these units. During the daytime shift, team members are assigned to inspect these units on a daily basis, focusing on aspects such as the nitrogen pressure, the integrity of the insulation, and the proper condition of all related facilities. Especially during strong wind conditions, due to the outdated insulation systems in Facility North, many of the insulating sheets have become severely corroded, which increases the risk of them being blown off by the wind and causing injuries. Every time a warning of strong winds is issued, an inspection of the insulation in Facility North is carried out to prevent accidents. (IV) Other aspects: 1. Coordinate with various functional departments to prepare relevant documents, implement all the required corrective actions on site, and ensure successful completion of inspections and evaluations such as those conducted by the China Chemical Safety Association, the three-system certification process, assessments related to industrial transformation and upgrading, as well as applications for the company’s qualifications for importing crude oil. 2. To improve the efficiency of identifying and addressing issues on a daily basis, the workshop has established HSE WeChat and QQ groups, using these online platforms to share company notices, requirements, and policies. This allows company documents to be communicated to employees promptly, enabling them to stay informed about the company’s management developments in a timely manner. 3. Before starting operations in 2017, the workshop strictly followed the company’s PSSR checklist, assigned tasks meticulously, and carried out multiple levels of review and inspection. By addressing the identified issues, it ensured the successful commissioning of the facility from the first attempt! II. Existing Problems and Shortcomings (1) There are still many instances of leaks at the equipment sites, especially in the heavy oil and steam systems; frequent seal failures pose significant challenges to on-site management. It is necessary for the workshop to take a long-term approach to reduce and eliminate such problems. (II) HSE maintenance is generally carried out in accordance with the company’s regulations, but there are still cases where maintenance records are incomplete or not filled out on time, indicating gaps in management and a failure to truly raise employees’ safety awareness. (III) There are also some shortcomings in training or activities that involve on-site drills. For example, when carrying out process-related tasks, the required personal protective equipment is not always worn, which could lead to unnecessary injuries in the event of an actual accident. It reveals that employees’ ability to handle safety situations needs further improvement. (IV) In terms of safety culture construction, the workshop still needs to explore new approaches, aiming to enhance employees’ safety awareness by creating a positive safety culture atmosphere. (5) Employees lack the capability to conduct risk analysis, and are unable to properly identify various risks in operations; as a result, their work is carried out blindly without an understanding of why certain actions are taken. This is a serious issue that reveals gaps in workshop training, and further improvements are needed in training management. (VI) The 6S management in the workshop area needs further improvement; the requirements and standards for the visual management of various equipment and facilities must be raised, as the current standards of having the bottoms of trenches and shafts clearly visible do not meet the company’s requirements. III. Work Plan for 2018 In light of the aforementioned shortcomings and issues, the workshop needs to establish clear measures to address them properly and ensure the safe and normal operation of the equipment. Therefore, the following work plan is formulated: (1) Thoroughly review the maintenance plans, record the areas at the site where leaks are likely to occur, discuss these issues with similar equipment in the surrounding area, and develop remediation plans to eliminate such problems at their source, while also ensuring that proper maintenance is carried out. (II) Continue to make greater efforts to enhance employees’ safety awareness, seek appropriate methods through communication, organize relevant training, and truly ensure that employees adopt a mindset of \"I want to be safe.\" (III) Foster a strong safety culture by setting up safety information boards in the workshops, highlighting the positive attitudes of employees, recognizing \"Workshop Safety Stars\" as role models, and thereby motivating everyone to be more proactive. (IV) Provide targeted training on risk analysis for all employees to enhance their ability to identify risks and ensure the safety of various operations at the facilities. (5) Continue to increase the frequency of drills related to on-site response plans, ensure that such drills are conducted at the frequency required by the company, and enhance employees’ capabilities in handling emergencies. (VI) Combine processes and equipment to improve management of the workshop site, raise management standards, and create a professional working environment. (VI) While focusing on one’s core responsibilities related to safety, it is necessary to learn about the various inspection requirements for equipment and processes. This helps to improve the ability to identify problems, as well as to detect potential hazards and shortcomings on site. With the care and support of our leaders, the workshop managed to get through the extraordinary year of 2017 safely. We achieved certain results, but we also identified many shortcomings. In future work, we will continue to pay close attention to safety measures, reinforce safety training for employees, raise everyone’s awareness of safety, and better fulfill all the production tasks assigned to us by our superiors. We will ensure that safety responsibilities are properly addressed, work with dedication and precision, and carry out all tasks responsibly, thereby contributing our sincere efforts to the company’s development.
