Based on our many years of experience with methanol-to-hydrogen plants and the analysis results you provided, the most likely cause is chloride ion corrosion. Check the levels of anions and chloride ions in methanol, and verify whether the conductivity testing for deionized water is ready. Generally speaking, deionized water is more likely to contain chloride ions
It’s most likely that the water quality is poor, with too high ion levels. When using a heat exchanger, care must be taken to avoid dry burning; the operating sequence must not be reversed
