Is the Hazop analyst primarily responsible for safety incidents of the device under analysis? For example, if he fails to analyze a certain condition, such as a specific temperature level, and no control measures are put in place in the design, then if an accident occurs due to overheating, is the responsibility lies with the design or is it shared between both?
It’s impossible. At most, it is just one of the indirect responsibilities for the accident.
The main party bears no responsibility. First, a HAZOP analysis is not the responsibility of just one person; in principle, the person in charge’s role is merely to guide the process – they should not participate in the actual analysis itself. Instead, they are responsible for summarizing the analyses conducted by experts from various fields, with the whole team working together through brainstorming during the analysis. Second, no analytical method can guarantee that all security issues will be analyzed; this is determined by philosophy, and no one can provide such a guarantee. So when a problem occurs, blaming the HAZOP analysis is both ignorant and foolish.
