Where can I find information on the relationship between pressure, flow rate, volume flow, and pipe diameter for various media (gases, liquids, vapors)? Are there any standard specifications?

You asked too simply. Either you state the medium and operating conditions directly, or that’s all anyone can do to answer. After all, this involves too many things. It gets complicated, with factors such as multiphase flow mixtures, pipe resistance, and molecular internal energy. If it’s just for explanation, one could write a doctoral thesis. It is recommended to ask more in detail, or include photos and such, to facilitate understanding and discussion among everyone.
