This is a broken coupling screw, and the break location is the same. Like shear stress, the diaphragm is not damaged. If it is shear stress, why is it concentrated at that location on the screw? What causes shear stress to be so high? Or, what causes the coupling to break?
What are the coupling’s alignment data, radial and axial tilt?

Is the pump shaft a little bent? Is it stable to rotate? You can consider it and look at the axis.
If you perform such an inspection and see that the corrosion is serious, will you not clean it? This is a stud bolt. If it is corroded and not cleaned, uneven stress is inevitable. It is recommended to do the things in front of you first and then analyze them.
In the past, the bolts of our wind turbine were broken because the maintenance personnel installed the bolts in the wrong direction.
It is very common for problems with alignment to damage the diaphragm. This has also happened to one of our vertical pumps, but only occasionally. Adjusting the alignment will solve the problem.
It is also possible that there is a gap between the nut and the washer when tightening the bolt, and occasionally a few are not tightened.
Shouldn't they be breaking a few at once? That's too dangerous
