How are the foundation bolts for tower equipment installed?

Are the anchor bolts embedded and fixed in the foundation during the civil construction of the tower foundation, with their larger ends facing downward, integrated with the concrete? If not, how is it done? If so, could an error of ±3 mm in the chord length between any two of the foundation bolt holes of the tower equipment cause the foundation bolts to not fit into those holes?
It’s embedded, combined with the concrete; there is a bend at the bottom of the foot bolt, and sometimes an additional horizontal rebar is added as well. The placement of anchor bolts is supported by molds, so construction errors are less difficult to control.
If the diameter of the anchor bolt seat cover plate is 30, with M27 bolts. If we assume that there are no errors at all in the construction of the bolts for the civil works, but the position of one of the foot bolt holes in the tower equipment is offset by 2.8 mm (within the required 3 mm), then won’t it be impossible to fit it in?

Let’s put it this way: if it can’t be fitted in, someone on site will use an oxygen cutting torch to cut and repair the problematic area. It’s not ideal, but it’s possible to control that during the manufacturing process.
It shouldn’t be the case. It’s similar to a flange, except that one piece is embedded inside; bolts are used to secure it, while the other flange is located at the bottom of the tower. Once they are lifted together using a crane, it’s no different from installing ordinary flanges. Although I’ve never seen such equipment before, I can still imagine how it works. :lol
