Problems in boiler strength calculation

I would like to ask everyone: when calculating the strength of a boiler’s shell, is it necessary to assume a wall thickness first? Should the hole diameter be taken based on the pipe diameter, or should it be 1–2 millimeters larger than the pipe diameter?
When calculating the wall thickness, a thickness must be assumed, and calculations are checked at this thickness until a determination is reached. For the requirements regarding openings in shell-and-tube boilers, please refer to Appendix A of GB/T16508.4
