During the major maintenance of the facility, a hot work permit was issued, and on-site inspections confirmed that the conditions for carrying out hot work were met; by this time, the welder had already reached the highest point of the facility to begin working. The patrol supervisor noticed oil in the pit beneath the cutting site where the light hydrocarbons were being removed. Although fire trays and fireproof blankets had been used at the site, the supervisor still felt that the safety measures were insufficient, so he immediately used a loudspeaker to order work to be stopped. At the same time, contact the construction unit to come to the site to pump out the oil, and use oil-absorbing felt to remove all the oil from the pit. The construction unit has also raised its awareness and further strengthened fire prevention measures.
During major maintenance operations, the site is large and the noise from construction activities is high; providing walkie-talkies for the inspection supervisors is meant to ensure that the construction workers can hear instructions clearly, thus keeping safety always in mind.
This issue should be one that the person responsible for handling work orders raises as a point for improvement; however, equipping supervisors with walkie-talkies is an excellent idea that worth adopting*
