Before applying at the counter for a license to operate as a handler of LNG hazardous chemicals, the applicant must first complete an online registration process; the application form must be filled out and printed online. The applying entity must submit the following documents to the municipal work safety supervision bureau in order to obtain a “Hazardous Chemicals Business License (Type B)” (three copies of each document are required, along with one electronic version; all documents should be in A4 size, bound together in order, with the cover labeled as: “Application Materials for XXXX Company’s Hazardous Chemicals Business License (Reapplication/Replacement)”): 1. Application form for a Hazardous Chemicals Business License, or application form for reapplication or replacement of such a license; 2. Copies of the business license issued by the administrative department for industry and commerce, or the “Enterprise Name Approval Notice”; 3. Safety assessment reports prepared by a safety assessment agency with qualifications at the provincial level or above; 4. Copies of the fire safety inspection documents related to the premises used for business and storage; 5. Copies of documents proving ownership or lease agreements for the premises and facilities used for business and storage (such as property ownership certificates, state-owned land use certificates, collective land use certificates, land use agreements, and construction project planning permits; registered lease contracts); 6. Copies of certificates confirming that the key personnel, supervisors, safety management staff, and employees have received the necessary professional training (if the legal representative is a foreign national and cannot attend the training, a copy of their identity card must be provided); 7. Well-established safety management regulations and standard operating procedures for safe work; 8. The company’s emergency response plan for accidents; 9. For hazardous chemicals that are classified as controlled substances or Category I precursor chemicals, copies of approval documents from the relevant authorities are required; 10. For companies that own or rent premises for storing hazardous chemicals, copies of safety assessment reports for those premises, as well as valid documents proving that they comply with legal requirements (such as safety registration notices, safety permits for hazardous chemical projects, etc.). Copies of certificates showing that the key personnel, supervisors, safety management staff, and employees have received the necessary professional training are also required; 11. Copies of lightning protection inspection reports and certificates for retail and storage facilities; 12. Copies of the authorization letter for the safety assessment and the contract related to it; 13. Actual photos of the company’s business and storage premises.
