On the afternoon of June 25, the fifth working group of the Guangdong Provincial Emergency Management Department conducted a follow-up inspection of the construction site of a petrochemical project in Guangdong, where serious issues were identified in project management during an inspection on the 22nd. Upon arrival, the working group found that the construction site was still in a state of chaos, with material transport vehicles, ordinary private cars, motorcycles, electric tricycles, bicycles, and workers walking home all moving in and out freely around the project entrance and exit ； The “three connections and one leveling” work at the construction site was carried out in a perfunctory manner; the project has only one rough, muddy road available for people and vehicles to use. The road that should have been used as an entrance/exit for pedestrians to separate traffic flows has not yet been completed ； Upon entering the construction site, it was found that the violations at the site, which were identified during the inspection conducted on the 22nd by Working Group 2 and for which Jieyang City issued notices requiring the company to make corrections, still remained unchanged ； When the working group requested access to project-related documents, the owner, the general contractor, and the subcontractors delayed things under various pretexts. Some members of the general contractor’s team even stopped the person in charge of the documents from delivering them to the working group, saying, “There’s no need to create trouble.” Through two inspections, serious safety hazards and problems were identified in the project’s construction management: first, the project was assigned to contractors without proper oversight; the construction project involved over 100 contractors and 23,000 construction workers. On-site inspections revealed that the project owner failed to bring the safety production efforts of the general contractor and subcontractors under unified management; no safety management agreements were in place for the concurrent operations carried out by various contractors, and the boundaries of responsibility were not clear ； Safety and technical instructions were not provided based on the risks associated with different construction projects ； Regular inspections of the general contractor and subcontractors’ work in terms of safety production have not been carried out ； Failing to urge the general contractor and subcontractors to promptly identify and rectify potential hazards. Secondly, the on-site management is in a chaotic state: the construction site is not enclosed, there are no warning signs, and people and vehicles can enter and exit freely without any inspection ； Standardization measures for construction sites, such as the implementation of the \"seven signs and one map\", separation of pedestrians and vehicles, and zoning of materials, have not been enforced. Building materials are piled up in disorder, and pedestrians and vehicles compete for space on the roads. All gas cylinders used for welding lack caps, creating a situation full of hazards and dangers at every turn. Third, there were numerous violations at the site; some employees in the construction area were not wearing safety helmets ； Lifting operations are carried out without any enclosures or supervision, and people walk around freely within the radius of operation of the crane arm ； At the scene, 2 workers were found standing inside a defective lift cage being lifted by a regular crane. Fourth, safety education is merely a formality; there are no records such as safety training materials or examination papers for the main contractors and subcontractors ； The training and education content consists only of theoretical knowledge, with no practical training or related facilities ； A random check of 1 employee on site revealed a lack of basic knowledge regarding safe production. Fifth is working on special tasks without the required licenses. During inspections of the high-density polyethylene plant’s construction site on the 23rd and 25th, it was found that two employees were performing welding tasks without having the necessary welder’s certificates ； On the 25th, a random inspection showed that the scaffolders working on the ethylene plant facility also did not possess a high-altitude work permit. In light of the above circumstances, the working group demanded on-site that the local cities, counties, **, regulatory authorities, and management agencies take legal action to investigate and deal with the matter. Immediate interviews were conducted, a warning letter was issued to a petrochemical company in Guangdong, and the person in charge of that project was summoned for a discussion. Immediate suspension of operations is ordered; the high-density polyethylene project areas where potential hazards are severe, as well as the caisson construction areas along the roads surrounding oil refining projects where minor accidents are likely to occur, must be shut down for rectification. Cases are filed for investigation; any illegal activities such as working without a license or performing tasks in violation of regulations at construction sites, which are confirmed to be true after investigation, are subject to legal action.
