Do anyone have any good suggestions regarding risk assessment reports? The risk assessment reports prepared in the past always seemed to lack a comprehensive format. I hope everyone can come up with more ideas; I want to create a more complete sample of a risk assessment report

Equipment operations, functions, and other aspects vary greatly; our company has risk assessment reports for at least 5 major categories of equipment. Our report includes several aspects for reference: scope of application, basic design parameters, operating conditions, potential hazards, failure modes and preventive measures, risk identification and mitigation, response measures in the event of minor leaks, major releases, and fires, as well as personal protective equipment and measures. The challenge is to fully identify the failure modes and to ensure that risk prevention measures are practical and effective.
