Some gasification furnaces locate the quenching ring a bit further back from the slag outlet, in order to prevent the high-temperature slag from directly eroding the quenching ring. But could this result in a shield of slag forming in front of the quenching ring, thereby hindering its cooling through water spray?
If the slag outlet is washed away, it could lead to the situation you mentioned.
