Is it a big problem if the sewage outlet, pressure gauge port, and safety valve port are in the wrong positions? Also, does an incorrect orientation of the level gauge have a significant impact? The import and export angles used to differ by 180 degrees, but now they are at the same angle. Is the equipment broken?

Formulate a renovation plan and have the design institute confirm it. Restart according to the design drawings, reinforce the incorrectly opened area, and be careful not to overlap the new opening with the welds.
You’ll need to revise this estimate. Generally, the pressure gauge should be placed at the top; it’s not acceptable for the level gauge to be in a horizontal position. The drain outlet should be at the bottom, while the safety valve is usually placed at a higher position, for safety reasons, to prevent spraying of liquid on people during startup. If you don’t want to redo it, then weld the faulty opening back together, conduct flaw detection, and open it again.
It is acceptable if the design unit gives written consent.
It doesn’t conflict with other nozzles; and with approval granted, the pipe flange and blind flange are welded together to serve as a spare port
If it’s necessary to proceed despite the errors, and if the size and orientation of the openings have changed, a rectification plan must be devised and then approved by the user. Generally, it won’t need to be scrapped.
Just find a designer to modify the image. If a design cannot handle even such problems, it is either because the designers are not competent enough, or it is done intentionally to make things difficult for the manufacturers.
