What equipment-related documents are required when applying for a pressure vessel manufacturing license?

1. The documentation is almost ready; I just don’t know if a certificate confirming that the equipment meets the required standards needs to be attached to it. Those who have dealt with this before, could you please send me a photo or the relevant form? Thank you in advance.
I didn’t explain myself clearly; by equipment, I meant things like lathes, welders, sheet metal rolling machines, and the like. It does not refer to special equipment
In use, pending repair, under repair, scrapped.
