Analyze the reasons

Could everyone help analyze the reason? There is a leak in the heat transfer oil steam generator on the client’s side (water inside the shell side is leaking into the tube side through the welds in the tube sheet). The welds in the upper half are fine; it’s only the welds in the lower half that are leaking. What could be causing this? Also, in the lower part of the tube box photo, the paint has peeled off and turned white (the whitened area is at the inlet for the heat transfer oil)

We also suspect that improper customer operation and a lack of water in the casing are to blame
