On a vertical heat exchanger, delamination is present in the range of 40–50 mm below the upper tube sheet, as shown in the figure below. Have you encountered it?
Is there a layer of stainless steel lining inside the tube? It seems that it was initially caused by corrosion; there is a state of corrosion present, and the quality of the pipe itself is also substandard
No lining, pure carbon steel seamless tube.
It’s hard to say; it depends on the method used for tube expansion – could it be that mechanical expansion has caused hardening and delamination of the surface layer of the heat exchange tubes?
The peeling area is not yet at the crimp joint.
Specific operating conditions? Shell and tube side fluids? Temperature and pressure?
OP, please specify the operating conditions and the fluid in the tube side. From the photo, it looks like damage and peeling of the inner coating.

First, you need to check the temperature of your medium; second, you need to verify whether the pipes are of satisfactory quality.
What temperature level can cause such a form of damage? Have there been similar cases?
