Format for process change description

Recently, the design institute submitted the drawings after completing the design work. However, some of our units’ equipment requires relocation or addition, and the design institute has requested a change report. Could that senior engineer provide a template for the change report to be submitted to the design institute?
Follow your organization’s requirements regarding faxes or other documents; simply clarify what needs to be done by the design institute and the conditions that must be provided to it.
No template is needed for this; just clearly explain the issue that needs to be addressed, write down the name of the organization, sign it, and include the date – that’s all.
If your company doesn’t have a specific document format for this purpose, you can simply write it as a notice or request for leave – with a title, main body, signature, date, and stamp, and that will be fine
Explain your requirements based on the actual situation and send them to the design institute. The design institute will respond to you in accordance with the standards: it can be modified or not, but the **standards must not be violated.
An engineering liaison form will suffice; if the written description is not clear, a schematic diagram can be provided
