What does the number 13 marked at the large and small ends represent?

It seems to be a dimension specification; the two crosses correspond to the centerlines of the thick and thin pipes, meaning that the height difference between the centerlines of the thin pipes at both ends and the thick pipe in the middle is 13 mm. The pipes are locally thickened here to ensure that the insertion depth of the thermometer meets the specification requirements.
The eccentric reducer features a height difference of 13 mm between the centerlines of the pipes at its two ends. For concentric reducing tees, the height difference between the centerlines of the two pipes is 0.
It’s probably the central deviation related to the size of the head
