Regarding the white crystals carried away when medium-pressure superheated steam is vented

During an unplanned shutdown at our plant, white crystals were found at the vent for superheated medium-pressure steam during the restart process; testing confirmed that these were trisodium phosphate. During normal operation, the phosphate levels in the boiler water remain within acceptable ranges. How can this problem be resolved, and what preventive measures should be taken in future operations?
There are no photos, so it’s not clear where it is. Please feel free to upload photos. The result indicated trisodium phosphate – was it added in excess during the softening process and ended up in the boiler, or is there some other reason?
