1. Phenomena: (1) The wire boxes are embedded too deep, resulting in varying elevations; there are gaps between the panels and the wall, the panels are contaminated with paint, and they are not straight. (2) The wire box contains mortar and debris. (3) There is a series connection among the phase wire, neutral wire, and PE protective wire of the switch and socket. (4) The wire ends of switches and sockets are exposed, the fixing bolts are loose, and there is insufficient length of wire inside the box. 2. Cause analysis: (1) The wire boxes were not fixed securely during embedding, resulting in template expansion and inaccurate coordinates during installation. (2) The construction workers lack a sense of responsibility; they do not fully understand the importance of safe use of electrical equipment and seek convenience over safety. (3) There is unreasonable close coordination among civil engineering specialists to ensure accurate and reliable fixation of wire boxes ； When the embedded wire box is too deep, an additional wire box should be installed. When installing the panel, it must be placed horizontally and vertically; use a level to adjust it so as to ensure uniform installation heights. Additionally, after installing the panels, the gaps must be filled thoroughly; no gaps are allowed, and the panels should be kept clean and protected. (2) Strengthen management and supervision to ensure that the phase wire, neutral wire, and PE protective wire in switches and sockets are not connected in series; first, clean out the mortar inside the box. (3) Fix the size during wire stripping to ensure uniform wire ends, with no exposed ends after installation ； At the same time, in order to firmly compress the wire, the single-core wire should be bent into a double strand when inserted into the wire hole, and tightened with screws. (4) There should be a certain amount of slack in the wires inside switches and socket boxes; 100–150 mm is generally appropriate ； Unreasonable attempts to save material at the expense of quality must be firmly eliminated.
