What accidents have been caused by instrument problems in hydrogen production units? Which aspects are more common: instrument failures, interruption of purge air, control instrument regulating valves, interlock instruments, etc.?
We have encountered purification air system failures; UPS-related failures have occurred in other shifts, while interlock failures have taken place in nearby workshops.
The purification air supply is stopped for a long time or for a short time! UPS backup power failure? It’s not used only when there’s a power outage; how could it malfunction under normal conditions?
The UPS failure occurred almost four or five years ago. At that time, I wasn’t focused on my work; I can’t remember the exact reason. All I recall is that the accident was attributed to a malfunction in the UPS’s instruments. There was an accident in the heavy oil workshop about seven or eight years ago, also due to instrumentation issues – all the DCS screens in the control room went black.
It’s not scary when instrument problems occur frequently; inaccurate readings or sudden increases or decreases in values are not a concern. What’s worrying is the interlock mechanism associated with these instruments, as it can potentially affect the operation of large-scale systems and equipment
False level signal of the instrument causes the low-level interlock
The gauge failed to communicate with the internal control system to disable the interlock, which led to improper operations in the control valve room due to local interlocks; this caused the furnace to shut down. False liquid level signals were transmitted, resulting in pressure surges. There are many possible consequences~~~~
